Probability of making mistakes is everywhere, if that occurs a proper root cause analysis to be done. Post that good knowledge transfer where it requires, fix the process loopholes if any, establish controls, these are key areas that a lead has to focus on. Mistake can happen but shouldn't be repeated.

I think it is important to understand why that person is making the same mistake repeatedly. Is there a gap in communication or the way training has been imparted? Is there a mismatch in the expectations set for the resource and the skill level? It may be good to sit 1:1 with the person and go over the entire process/ task. Observe the areas where the errors occur and then coach.

You’ll want to sit down with them. Get to the cause of the problem. The person may not have the training to do that or the person may be checked out. It starts with taking the time to sit down with them an address the problem.
