Thrilled to share a new manuscript about how CMS CLIA regulation could be adapted to provide a framework for essentially all hospitals and a large number of clinics to conduct their own internal AI quality management https://lnkd.in/ebGkxUNP A few neat things about this paper: 1) the authors came together online through LinkedIn. I started messaging with Brian Jackson ~6 months ago about how CLIA and industry standards for clinical laboratories could address many of the concerns raised around AI implementation in healthcare. We brought together a distributed team to work on the paper and many of us have never met in person. 2) CMS' CLIA program covers 320,000 laboratory entities in the US. That's a big number and it's the order of magnitude we need to be thinking about when we talk about building capabilities at a national level to promote the safe, effective, and equitable use of AI. You can't go from 3 'quality assurance labs' to hundreds of thousands. From the beginning, we need to be thinking about scalable approaches that can support diverse settings. 3) Everyone is focusing too much on FDA. There's broad agreement that there's a gap when it comes to post-market performance monitoring of AI systems. Rather than framing that as a gap in FDA oversight, we need to be looking more to the CMS CLIA policies. CLIA complements FDA authorization by providing a path for post-market performance monitoring and close quality management of AI. 4) The paper really goes down the rabbit hole. Check out the table in the comments. We really dug into all aspects of CMS' CLIA program and specified how the program could be adapted. It was amazing to work with a true expert in clinical laboratory operations (Brian Jackson) to put this together. We want to emphasize that there is not a need to start from scratch to build something completely outside of existing government and regulatory structures. 5) Our proposal has nothing to do with the VALID act. We mostly stay clear from the debate surrounding laboratory developed tests (LDTs). 6) We're not selling a silver bullet. Equipping all healthcare delivery organizations to internally manage AI will not be easy. Adaptation of CMS' CLIA program would also require government supports. Califf spoke with NPR about the agency's process for regulating artificial intelligence in healthcare during which he stated, "It's very important that we get the clinical community - the health systems, the professions, the people that you see for your health care - to be very involved in developing what we would call an ecosystem of regulation where these algorithms can be constantly evaluated." In our Mayo Clinic Proceedings article, my colleagues discuss ways in which internal accountability for safe, effective, and ethical implementation of AI can be embedded into healthcare institutions.
