Every ardent structural engineer tries to cover all uncertainty factors with the factors of safety but still there can be unforeseen and unpredictable integrity issues.The solution to this lies in sharing the issue thread bare with the client,the possible fallouts and the possible solutions to these and the final solution out of the feasible ones should be the one which satisfies and gives a rational confidence to the client and the engineer alike.There can be no invincible problem in engineering and also there can be no pre-defined best solution.But the best is the one which satiates the confidence of the agency & the client at the same pitch.This also means that one should not give up in such situations as there lies the real innovation!

Inappropriate use of machinery to get fast results and many more However these issues can be many times Dealt merely by vigilance and monitoring but sometimes even after proper due diligence and caution, issues can go beyond control due to other factors like temperature, rain etc It is recommended to duly checking structures integrity with a detailed checklist during execution.
