Absence of cohesion could indicate that all are not aligned to the common problem that is being solved and the overarching goal. There is no big motivator than this. When everyone is aligned on this, lot of things fall in place. Next step involved is having a clear responsibility and hierarchy. I have been in teams where suddenly one will walk and say that they are your superior. You can imagine the confusion it can create to the teams. If both of this are addressed effectively, you could see cohesion issues reducing significantly. Beyond this, it can be a specific team/ person related, which warrant a at depth discussion to get the root cause out.
