Through Care Beyond Walls, a pilot project with AWWA Ltd, we’ve been rethinking dementia care by creating meaningful connections between seniors and the community. Since 2022, this initiative has empowered seniors at AWWA’s Dementia Day Care Centre in Ang Mo Kio to engage in interest-based activities, participate in community outings, and stay connected to life beyond the care center. By partnering with organisations across sectors, such as SOSD, Kong Meng San Phor Kark See Monastery 光明山普觉禅寺, Elias Park Primary School, Sri Darma Muneeswaran Temple, and Willing Hearts Kitchen, we’re building a dementia-friendly network that not only supports seniors but also engages their skills and cognitive abilities. Together, we’re shaping a community that values inclusion, well-being, and quality of life. ⭐ Watch Care Beyond Walls in action 🎥: https://lnkd.in/gkKz_m26 #CareBeyondWalls #PersonCentredCare #SilverBrigade

On 9 October, we held a panel discussion at our office to launch the findings from "Pulse on Palliative Care." This survey, the first of its kind, revisits public knowledge on palliative care, a decade after our 2014 Death Attitudes Survey. Following a presentation of the results by Kantar, Drs. Neo Han Yee of Tan Tock Seng Hospital, Alethea Yee of Lien Centre for Palliative Care, Shirlynn Ho of National Cancer Centre Singapore, and Ng Wai Chong of Home Nursing Foundation, shared their insights on why proactive palliative care is intrinsic to good care. The conversation focused on breaking misconceptions and addressing suffering, with the goal of making palliative care more widespread in Singapore. As we work to change the narrative, stay connected with us for more updates on how we can improve care for everyone. The Post Diagnostic Support (PDS) programme, first launched in 2020, has made incredible strides since its inception. As we continue this journey with PDS Plus, take a moment to hear from those personally involved in this reflective video: https://lnkd.in/g7HvMH9M
“Dementia Singapore is stepping up to be the leading domain expert in providing community palliative care for people living with advanced dementia,” shared Jason Foo, CEO of Dementia Singapore. He was referring to plans to aid a targeted segment of the dementia community as part of Dementia Singapore’s latest Post Diagnostic Support (PDS) Plus programme, funded by Lien Foundation. He noted some of the current gaps in the system, from the limited support in the transition from daycare to home care for persons with advanced dementia, to the lack of engagement when planning for future needs and capability in management. But he also affirmed that while information and resources on palliative care are currently fragmented, “we are learning to make things better”. Dr Philip Yap, Senior Consultant, Department of Geriatric Medicine, Khoo Teck Puat Hospital and Chairman of Dementia Singapore, joined in to share how important palliative care is for people living with dementia. He stressed the need to provide for the person’s total needs and care, while upholding the person’s dignity and maintaining quality of life. The phrase Yap chose to emphasise is “person-centred care”, adding as well how communication is key to the goal of empowering those impacted with knowledge.

A dementia diagnosis need not mean the end of life as you know it. Since 2022, our Care Beyond Walls project, in partnership with AWWA Ltd, has been breaking boundaries to design a new form of care for persons with dementia. Our approach focuses on personalised, interest-based activities and regular opportunities for our clients to venture out and be supported by the community. During our recent visits to host organisations, we saw firsthand how our seniors are making meaningful contributions at SOSD, Kong Meng San Phor Kark See Monastery 光明山普觉禅寺, Elias Park Primary School, Sri Darma Muneeswaran Temple, and Willing Hearts Kitchen. Our project aims to challenge conventional dementia day care services, enable persons with dementia to use their skills and cognitive reserves, and help them stay integrated in community life. We’re identifying and building supportive community partners, deepening Person-Centred Care practices, developing services for caregivers, and creating a dementia-friendly community around AWWA's existing Dementia Day Care Centre @ Ang Mo Kio. Together, we're creating a community of care that supports our seniors' well-being and integration. #CareBeyondWalls #DementiaCare #CommunitySupport #PersonCentredCare #ActiveAgeing

Helping patients help themselves often means reconnecting them with feelings of amazement, wonder, and hope—emotions that can fade after prolonged illness. Non-pharmaceutical interventions like magic can reignite these feelings, significantly improving overall patient well-being. Even when patients know it's a magic trick, the feelings of hope and joy remain undiminished. The real magic lies in the effort and care behind each trick, creating a sense of connection and appreciation. This result is due to the dedication of healthcare professionals at Tan Tock Seng Hospital's Department of Psychology and Geriatrics Department. Their willingness to learn new skills to engage their patients demonstrates profound kindness, care, and support! After all, kindness is the purest form of magic. ✨ Let's celebrate these innovative approaches to enhance patient well-being and transform healthcare! ❤️💭Mental health and well-being is something I care deeply about. And as PM Lawrence Wong said, this is a national priority. That’s why one of the key work of our Inter Agency Taskforce on Mental Health and Well-Being, is to strengthen community mental well-being. And one of our key partners is Limitless, who has been training youth peer supporters at its Peer Support Summit this week. 🧍Peer supporters are the first line of support for their friends. And the training conducted by Limitless and Care Corner equip our young peer supporters to help their communities maintain good mental well-being. 🙏 ✨A shout-out to the mentors, facilitators, and partners as well, for your time and effort in making the summit possible! Let’s keep this momentum going as we continue to step forward, support, and empower each other to make a difference in the lives of our youths.
