Have an open and honest conversation with team members to understand the issues. Often times we repeat the project goals assuming a) everyone understands, b) is on board with it (you have buy in) c) is enthusiastic d) most often the case does not see whats in it for them. The engagements can be done as a team but identifying the root causes to mis-alignment and then having the team work out how to overcome these can be a good way to align. Stepping back and letting the team workout what to do is often hard as we always assume they are on board 100% of the time and know whats in the mind of the project. That is not always the case and working something out can help.

Ask WhY. Dont assume the reason of failure. Ask why first. Why were you not aligned? Once identified, work on that- it may be miscommunication, lack of communication, failure of some process, bad pizza. Solve for that issue with your team and evolve your process going forward.

Well, the first thing is to know the "Why". It can be technical issues, personal issues, and variety of problems that could be incomprehensible at first but are actually quite likely to happens. After knowing the "why", schedule a meeting to discuss and solve or acknowledge all of the issues above. In some case, it is not the project goal, but the way towards the goal that needed to be adjusted.
