What started with a commercialization project between St. Joseph's Healthcare Hamilton and Canadian technology company, PointClickCare, turned into an incredible, province-wide initiative improving the continuity of care for long-term care residents. Learn more about Project AMPLIFI below ⬇️
Project AMPLIFI: Integrating Healthcare Data for Improved Care Did you know that critical health information shared between hospitals and long-term care facilities often arrives as a stack of paper with the patient or via fax? This requires manual sorting and data entry into each organization’s electronic medical record system – an outdated process that can result in incomplete information, delayed care, and potential risks to patient safety. With support from Ontario Health | Santé Ontario, St. Joseph's Healthcare Hamilton led Project AMPLIFI, a digital health information interoperability initiative connecting hospitals and long-term care homes seamlessly, aiming to enhance patient care quality across Ontario. 📖 Learn more about Project AMPLIFI in our latest Annual Report: https://bit.ly/3ZpHINC #WeAreStJoes #DedicatedToDiscovery #ChallengeStatusQuo

After 12 incredible years at Trillium Health Partners (THP), it is with a mixture of gratitude and excitement that I share I will be stepping down from leadership roles as President, THP Solutions and Chief, Clinical Innovation on February 14, 2025. My journey at THP has been nothing short of remarkable. I’ve had the privilege of working alongside some of the most dedicated, innovative, and compassionate staff, clinicians, and leaders in the healthcare sector. Together, we’ve made a significant impact on the lives of our patients and our community, and for that, I will always be grateful. Whether it was my tenure as Chief of Staff, leading the expansion of the hospital’s education program or supporting the creation and growth of a first-of-its-kind operating model that aligns research, innovation and revenue diversification for THP, I am incredibly proud of what we have accomplished, together. I’m especially proud of the work we’ve done to build the foundation for the next generation of healthcare leaders at THP. As I take on this new chapter, I’m excited to see the future of the organization continue to flourish under the leadership of those I have had the honor of mentoring and working alongside. As I shift my focus to this next chapter, I remain committed to improving health outcomes, leveraging new technologies, and strengthening our healthcare ecosystem to benefit both patients, providers and entrepreneurs. In 2019, I was proud to found, the CAN Health Network which represents a vision for not only the future of healthcare but our economy. It embraces innovation, collaboration, and positive change across the sector and recognizes the critical role that Canadian businesses play. I will also remain a professional staff member at THP, where I will continue to provide front line care to the patients and work along side some of the most dedicated health care professionals in Canada. In addition, I will remain an investigator at the Institute for Better Health and professor at the Temerty Faculty of Medicine at the University of Toronto. To all the amazing staff at Trillium Health Partners—thank you for embodying our values of compassion, excellence and courage, and demonstrating everyday what it means to be Better Together. I am proud of what we’ve accomplished together and excited for what lies ahead! To the patients and community, thank you for entrusting me during some of your happiest moments, and some of the darkest days, it is an honour of a lifetime to care for you and your family. Onward to a future filled with innovation, progress, and impact.

Did you know that diabetic foot ulcers affect 15 to 25 per cent of people with diabetes? 👣 These chronic wounds are often difficult to heal, with the current standard of care having only a 50 per cent healing rate after 12 weeks of treatment. DermGEN has shown an 80 per cent success rate in healing diabetic foot ulcers in just four weeks.
