BMHC Board President, The Honorable Dr. Larry Wallace, Jr., Ph.D., co-authored a health-equity-focused article with Dr. Norman Chenven, MD., of Austin Regional Clinic, and David Stein, CEO of Babson Diagnostics, called "Three Guys Walk into a Conference And...", in the July 2024 Edition of the Austin Medical Times Magazine. What is unique about this collaborative piece is how it was born. Three guys, who come from very different places—a medical group, community care coordination, and a diagnostics company—met as they all sat on a panel at the 2023 Healthier Texas Summit hosted by It's Time Texas in collaboration with the University of Texas System. However, they all had one question in common and that is: "How do we remove barriers, make access to care easier, and reduce any fear and anxiety related to getting medical help?" They all acknowledged statistics show the health equity gap appears to be more acute in Texas than in many other states. According to their article, our state recently ranked poorly across 24 healthcare metric determinants spanning across four categories: access, outcomes, cost, and quality of care. They also agreed the challenge of improving care is big and complex, but the place to start is with preventative care. Transportation barriers significantly impact healthcare access, leading to approximately 3.6 million people missing or delaying medical care annually. This challenge contributes to 25-50% of all missed appointments across health systems. At the 17th Health Disparities Conference at Xavier University of Louisiana, Audrey Shawley, MD, and Isabel Anderson unveiled their study titled “Impact of Free Rideshare on No-show Rates and Healthcare Utilization in High Medical and Social Needs Patients.” Their research at the MedVantage Clinic within Ochsner Health investigates how free rideshare services can mitigate transportation issues as a social determinant of health. Dr. Shawley, who is preparing for a residency in internal medicine, and Isabel Anderson, a third-year medical student, explore effective strategies to improve healthcare access. Early findings highlighted the vital role of communicating the availability of paid rides and the importance of attending appointments, supported by phone reminders.

The latest certificate-of-need drama from North Carolina, with Duke Health and UNC Health battling over who gets best positioned to grow with the fast growing Raleigh-Durham "triangle" metros. The State's CON (DCDHHS) once again ruled for the high-cost incumbents and gave no new capacity to community ambulatory care, where real competition can be fostered. The state's "need plan" says Durham/Caswell County needs 40 new acute beds and 4 new ORs, and 3 applicants came forward with proposals to meet that need--UNC, Duke, and an ASC called Southpoint. The NCDHHS determined that UNC was a new provider (really?!) and chose UNC to promote competition in Durham/Caswell where Duke currently dominates. The ASC got no new capacity. UNC has the upper hand as a state appeals court recently upheld the decision of the NCDHHS (and an ALJ) to award 40 acute care beds and 2 ORs to UNC, with Duke getting 2 ORs in its ASC. I continue to be amazed by the false precision used to determine how much new capacity is needed and where it's best located, and the state's application of criteria that favor the big expensive systems getting bigger. It isn't that common for two academic systems to compete in the same or adjacent markets like the Raleigh-Durham-Chapel Hill metros, and RAND price data suggests that Duke and UNC are competing on price to some extent, but their prices are still much higher than other community settings. Patients pay more for labs, MRIs, primary care and specialty care visits and delivering babies in AMCs compared to community hospitals or free-standing facilities. As KFF data shows, hospital markets where there are still 3 systems competing are becoming rare, and #NorthCarolina should be doing everything it can to preserve competition in the capitol region. 🚀Excited to share that I am currently working on my dissertation: "An Audit of Missed Appointments in Podiatry Services: A Comparative Analysis of NHS and Private Services." Missed appointments or DNAs (Did Not Attend) are a significant challenge in healthcare, impacting service efficiency and patient outcomes, especially crucial in podiatry where consistent care is vital for managing chronic conditions. 📊 My research involves an in-depth audit of missed appointment rates at both an NHS podiatry clinic and a private clinic. The aim is to understand the patterns and underlying causes of these missed appointments and to propose effective strategies to improve patient adherence and service delivery. 📚 I've immensely enjoyed reading papers related to this topic, delving deep into the complexities of healthcare delivery and human behaviour. This article sheds more light on the #healthcare access challenge we’re facing in the U.S. Expanding access to care is one of our highest priorities at Sutter Health so we can help more patients lead better, healthier lives and make it quicker for them to receive care. This includes adding clinical capacity by growing our workforce – including the number of #physicians affiliated with our system. We’re not only recruiting today’s physicians, we’re investing in tomorrow’s through a significant commitment to and expansion of our Graduate Medical Education programs, which will ultimately train 1,000 residents and fellows a year. We’re also expanding our presence in the communities we serve by building new care centers, enhancing our existing facilities to ensure they best meet the needs of those we serve, and using #digital tools to make our care more accessible. It’s a big challenge and there’s much more to be done, but we’re making real progress thanks to great work by teams across our organization and our focus on making it easier for patients to navigate their care and engage with us when, where and how they want!
