Ophthopedia Update:Aberrant regeneration of accommodation in pediatric inferior division oculomotor palsy: Aberrant regeneration occurs in forms of oculomotor motor nerve palsy and frequently involves the pupil, but the incidence and functional impact of ciliary muscle involvement in pediatric patients is sparsely reported in the literature. A 4-year-old girl presented with inflammatory oculomotor motor nerve paresis affecting the inferior division. Initial treatment focused on her inability to accommodate through her physiologic +2.5 D hyperopia and the prevention and treatment of amblyopia. She subsequently developed aberrant regeneration of the pupil, with miosis on adduction. #Ophthalmology #Eye #Ophthotwitter

Rebuild trust with our patients this summer...so we don't panic this winter. 🙂 Our focus for the coming months is to leverage our patients to solve the frustrations we have in the NHS. These are issues not insurmountable.. with their help. Patients who are confident in their local services, and their post surgical follow up care, seldom overuse emergency services. Lack of access through 'digital exclusion' however, dampens this confidence... We would like to encourage early communication with patients, even as we place them in intermediary care settings...patients who do not feel abandoned have much better outcomes! Myth: To treat constipation laxatives should be started, then decreased, and discontinued. Reality: Constipation is a spectrum and we, as pediatric colorectal surgeons, treat the worst type (severe ones) due to anorectal malformation, Hirschsprung disease, and Idiopathic Constipation. We tell parents and patients that it is a chronic condition, incurable but manageable. Laxatives do not cure the constipation but they allow patients to have daily bowel movements. 'Before a transition into hospice, a patient's journey is often long, convoluted and tiring. There may have been many years of active treatment, surgery, chemo, radiotherapy, to name a few. A journey that's filled with appointments, blood tests, scans, treatment and supportive care; critical moments with little time and space to fully process what is happening. Where active treatment is not an option at initial diagnosis, these patients are suddenly confronted with with an incurable illness and the unbearable realisation that their life will soon come to an end. These transitions into palliative care require sensitive end of life discussion with the patient and their family, where priorities and choices are made on how to live out the precious remaining time.... Clinical experience as a nurse and therapist has taught me that these sensitive conversation cannot be accomplished in a single session. It is a process requiring protected time and professional support... And this is where the role of complementary therapies such as aromatherapy has the potential to bridge this gap by easing the passage of transition into palliative care; to reconnect patients with what matters in their life; to empower them with approaches where they have a sense of control; and to support their loved ones across the entire illness trajectory and through the inconceivable loss that comes with the finality of death.' When it comes to babies and toddlers, parents are taught to be very careful about small parts, due to the risk of choking. But another risk that has to be taken in consideration is aspiration, as in this case where little Asher was thankfully saved 🙏 FBA (Foreign Body Aspiration) is when food, drink or other material accidentally enters the airway tube and travels down to the lungs, instead of entering the food tube and travelling down to the stomach. This can cause serious health problems, such as pneumonia. On one side it allows more time for treatment, compared to choking incidents, but on the other hand the symptoms may be so "silent" to make it difficult to be detected (especially if parents are not aware of what happened, with infants not being able to explain), allowing the infection to grow severe before getting treatment. Be very careful about food and objects that can be very small and light, which increases the risk of being accidentally breathed in. Examples: popcorn, tiny marshmallows, small round chips such as the cheesy ones, powdered sugar, small polystyrene foam pieces... A good way to easily assess the risk of aspiration is to place the small parts on the palm of your hand and blow very gently. If they move very easily, better to keep them away from your small ones (or mitigate in your safety risk assessment, if likely to enter in contact with infants or toddlers) #productsafety #childrensafety #aspiration
In the blink of an eye, a fun arts and crafts activity turned life-threatening for 7-month-old Asher. Asher was watching others play with art sand when the granules accidentally got in his mouth as he inhaled, making it hard to breathe. Asher’s mom, Taya, called 911 and a fast-acting cascade of life-saving care began. First responders were able to get Asher breathing again and he was transported to his local hospital. More specialized care was needed to help save Asher, and he was transported to Children’s Mercy. When Asher arrived, our surgery, cardiology, blood bank, ECMO, ENT and pulmonology teams were ready to spring into action. Asher was put on an ECMO (extracorporeal membrane oxygenation) machine, which functions as the patient’s heart and lungs in life-threatening situations. Putting a child on ECMO is not an easy decision. But Children’s Mercy teams worked for hours to clear the sand out of his lungs – thankfully, with great success. “It was not a given that this child would survive,” said Dr. Sarah Brunner, Medical Director, Pediatric Care Unit Quality Improvement. “It took every single person at that bedside to make this outcome happen, and it had to be done fast. It was very much a team effort and provided a real connection to purpose and to our true mission.” Asher has now fully recovered, returned home and is back to being his smiley, happy self! 💙

Cloacal Exstrophy in a nutshell for pediatric surgeons: 1) All bowel must be incorporated into the fecal stream and an end colostomy should be created (NOT AN ILEOSTOMY) shortly after birth. 2) The only way to know if a patient is a candidate for stoma pull through is by doing bowel management through the stoma. Clean/empty bag for 23 hours after enema = successful bowel management. 3) Bladder reconstruction should not happen before the decision of pull through. These patients need to be seen by a multidisciplinary team. 4) Fecal incontinence is not a contraindication for the pull through. Failure of bowel management through the stoma is. 5) Consider sending the patient to a specialized center with experience in cloacal exstrophy. Those are rare conditions but we owe to the patient their best opportunity to improve quality of life. CMS Requirement Emergency Preparedness Program The Ambulatory Surgery Center must establish and maintain an emergency preparedness program that describes the actions to be taken in an emergency and includes a process for cooperation and collaboration with local, state, and federal emergency preparedness officials.
