NYC is undisputedly the best place to build in healthcare AI 🏥 , sitting at the intersection of exploding tech/AI momentum + matured healthcare/life sciences economy, with all-time-high venture + gov funding + R&D scientists Had a ton of fun hosting an intimate healthcare-centric dinner tonight with researchers, engineers, founders, health system operators, and physicians from Tandem, Tennr, Mount Sinai Health System, Google DeepMind, Counsel Health, EliseAI, Neo, Weill Cornell Graduate School of Medical Sciences, Inductive Bio, NYC Health + Hospitals with John Bryson (Banc of California) Lots of excitement all around, coupled with some frustration with the deeply brokenness of U.S. healthcare - some of the hot takes / promising areas we talked about: - If your healthcare AI startup’s product is on Epic’s roadmap, you have no chance of winning, as health systems would much rather adopt an okay solution from Epic ⏳ - AI scribing tools have not meaningfully evolved since v1 📑 - Healthcare will get much worse before it gets better - the U.S. society is totally unprepared for the silver tsunami / rapidly aging population 👵 (Visible Ventures portco Grayce is tackling this problem!) - Better retrieval of medical records with precision on what part of a patient’s records is relevant to diagnostics will unlock huge time savings for doctors (and correct their human errors) 🔬 - Improving scientific reasoning abilities of frontier models will lead to better reasoning in diagnostic workflows, which is currently under-explored due to difficulty of finding high-quality benchmarks 🧠 - Startup founders can’t build a durable, venture-scale company on the sole basis of prior auth 🛣 - Machine learning models are speeding up DFT calculations significantly, enabling improved analysis and understanding of biomarker systems 👩🔬 - We will live in a future where pharmacy benefit manager (PBM) is obsolete 💊

That said, agents should be trained to handle very narrow tasks, reliably, first vs. an open-ended approach many startups are attempting today - Robotics: 🤖 While the science is not there yet, the robotics market and its impact will be much bigger than LLM. The introduction of humanoids will fundamentally change how we live. That said, the U.S. is in a vulnerable position due to lack of supply chain independence, compared to rivals like China - Value creation: ✨ AI personalization is overrated as consumers mostly care about status (i.e., ‘whatever is the best’) vs. what's tailored to them. Delivering great UX is still hugely undervalued, for both enterprise and consumer use cases. Cost and reliability remain enterprise buyers' front-and-center priorities. Hosted a curated group of NYC AI builders for dinner tonight, incl. founding ICs at Modal, Captions, Replit; founding GTM at Together AI, Databricks Mosaic Research; researchers at Google DeepMind, Harvey, GitHub; product leaders at Anduril Industries, OpenAI, Google DeepMind, Zapier; and founders from South Park Commons We talked about things like the human brain, the foundation model battle, re-imagining AI interfaces, future of consumer AI, super intelligence, AI's role in education, and self-driving cars. As usual, we asked each guest their AI hot take and predictions: 🤠 - OpenAI & other foundation models do not deserve their hefty valuations - a 6-month advantage over open-sourced models like Llama is not a sustainable value driver 🐻 >guest pushback: OpenAI will be a consumer business long term, and the future products layered on top will drive real value 🔮 - Most of the consumer AI as we know today is single-player - there’s tremendous potential in multi-player consumer AI 🤼 - Ads monetization will create perverse incentives for foundation model providers —> user experience with free models will worsen 🙃 - Super intelligence is overhyped, as every physical system has a limit (e.g., speed of light) ☄ >guest pushback: a 10% b2b productivity gain is still a massive value-add 🉐 - There’s nothing magical about the human brain - difficulty comes from learning how to control it with consistency, same logic with AI models for video generation 🧠 - The current AI ecosystem moves too quickly and cares too much about creating fancy product demos - instead, we should re-anchor around solving real user problems 🏃♀️ - Building very specific AI models that tackle very specific tasks is the future of AI 👩🏫 - The chat interface as UI/UX layer is extremely lazy 🐨 - As we approach AGI, Washington will feel pressured and incentivized to do what they didn’t do with social media to mitigate issues like fraud, lack of safety, etc. 🚨 - We should be losing our minds about self-driving cars instead of gen AI, which isn’t even that cool 🚙 >guest pushback: AI is more like a space race, not a bubble - GPS is now ubiquitous as a result of heavy investments in space; similarly, investing in AI will enable better self-driving cars 🛰 cc: Gopal Raman, John Bryson

Caregiving is larger than any single occupation in the U.S. It is the most significant and invisible work that we will ever undertake. By 2030, the population of adults over 65 is expected to surpass children, indicating a rise in demand for caregivers, just as society’s social safety nets—Social Security and Medicaid—are about to collapse. We need new solutions to bring and keep talented people in this field, like the one that Grayce is building. Today, we are excited to announce our investment in Grayce, a social care platform designed to address unmet needs in medical care. Rooted in a social work care model, Grayce supports family caregivers as they navigate caregiving complexities, reducing the stress and overall toll imposed on them. Julia Cohen Sebastien shared her compelling and deeply personal story below. Having grown up with a mom who ran hospice programs and having listened to her calls day in and day out with families, helping them manage through the highest-stakes moments for their loved ones, I always wondered how people navigated this caregiving challenge if they didn’t happen to have a friend like my mom in the right place at the right time. Today, as a mom of five kids, Grayce is squarely in the strike zone for our mission at Visible Ventures . This is a call to action that we will answer because investing in solutions today is our best hope for better outcomes for all of us in the future. Employers must address this pain point now.
