Great call from Dr. W who visualized minimal periosteal reaction on x-ray confirmed to be 2nd metatarsal fracture. The patient had history of pain for one week following trauma. [Images] XR foot demonstrating minimal evidence of periosteal reaction along the medial 2nd metatarsal, visible only on the frontal image (red circle). A magnified view of that image is also shown. The lower images are axial fat-saturated PD and sagittal STIR images from subsequent MR, which confirm the reaction at this level with incomplete fracture.
