As we welcome 2025, we’re taking a moment to appreciate the importance of caring for ourselves, each other, and our patients. A big thank you to Urban Roots, a community farm committed to providing equitable access to fresh food, for hosting our IT team for a morning on the farm during the team’s recent offsite in Austin. “We enjoyed the fresh air and team building, and the visit reminded us of how important it is to not only give back to our communities, but also to fill our own cups in the process. I’m proud to work at a company that is focused on helping people, both employees and the patients we serve, live healthy lives,” said Toya R., our IT application support supervisor who organized the visit. Here’s to a new year of growth, renewal, and reimagining healthcare.

As we turn the page to 2025, we’d like to take a moment to celebrate the hard work and many accomplishments of our colleagues and partners in 2024. Earlier this year, our CEO Robert Sepucha reflected on everything we have accomplished two years into pursuing our mission of transforming kidney care. https://lnkd.in/e8aVVua3 Since then, we have continued to build on our success: 🌟 Recognized as a top performer in CMS’s Kidney Care Choices (KCC) Model 🌟 Expanded our value-based care agreement with Humana to serve more patients 🌟 Launched Acumen Rounder to help nephrologists better manage patient workflows 🌟 Earned 3-year NCQA Population Health Program Accreditation Our work is not done. Cheers to a new year of working together to improve the lives of those we serve.

In all the big and small moments—from helping patients navigate the complex transplant process to sharing new healthy meal ideas—our team members touched so many lives this year. And then there is all the work that happens behind the scenes! Wishing all of our colleagues, patients, and partners comfort in what brings you joy this holiday season. Thank you for joining us on our mission to reimagine kidney care.

Our partners at Eastern Nephrology Associates, PLLC were one of three participants to achieve a perfect quality score in the government’s value-based kidney care model. In a new article for Healio, Dr. Carney Taylor MD, MBA, co-president of the practice, describes how the model is driving promising results and signals that investing in value-based care can consistently produce quality outcomes and cost savings. As we all try to envision what the healthcare landscape will look like in 2025, our CEO Robert Sepucha says we are at a “tipping point for real transformation” as payers evaluate data showing value-based care is improving outcomes for our most complex, expensive patient populations and plan for further investment in value-based models. As he shared with Health IT Answers, “we hear from provider partners across the country that value-based care is here to stay and they are all-in on this model.” Read his full prediction and what other experts expect we will see in 2025 in Answers Media Network: https://lnkd.in/dbbNdhJd

“In the past, we looked at diseases in silos. You have diabetes and the complication is kidney disease, or you have diabetes, and the complication is CVD. But that’s not what’s happening biologically. Biologically, you’re having parallel processes in different organs that probably have underlying pathophysiology in common…GLP-1s are working at a very fundamental level to alter this biology.” – Dr. Richard Pratley Great article in Healio capturing expert takes on the FLOW trial and GLP-1s as an effective treatment for CKD and type 2 diabetes. https://lnkd.in/eZDgJByW
