Simple way to avoid miscommunication is When ever there is a handshake pls ask someone from the team to note down the action points and assign a action owner and get a tentative date of completion. If the project is of long duration then better to have tracker , it could be excel or any other suitable tool. Update the tracker for updates date wise this really helps I have done on projects which were more than 6 months duration. Clear communication is key to success.
