DFMEA ( Design failure Mode and Effects analysis) is an excellent methodology to use in the development stage of any product or prototype. I would suggest repetitive DFMEA meetings with key stakeholders in order to mitigate risks and tabulate solutions

As one of the member write about DFMEA, yes this is important to overcome the issues from proto development. Also there is PTDB (past trouble data base) it is very useful to refer this. Issues occurred in past for similar parts and how we rectify them is recorded for each project.
