First of all, I conduct a through user research to understand behaviors, motivation and pain points, this deals with problems definition, goals setting and user persona creation. Then, I progress from low fidelity stretching to mid fidelity wireframing and high fidelity definitive prototyping. This process refines design concepts. Next, I conduct usability tests, gather quantitative and qualitative feedback and iterate designs accordingly as user testing and feed backs are crucial. Finally, I establish design systems, implement user feedbacks, heuristic loops and continuously refine designs based on user behaviors and feedbacks. I hope this helps?
