Dependencies are a type of risk. The title makes it feel as dependencies and risks are different. Change the title. Talking about dependencies. The goal of refinement is to clarify PBIs and make them small enough to fit within one Sprint. If there are dependencies between items, one of the goals of refinement (called grooming here) is to break these dependencies. A Product Backlog is expected to contain items that can be implemented independently from one another - if not there is a huge constraint in ordering the Product Backlog (which should be avoided as much as possible - probably not possible all the time though)

It is quite possible that things might fall through the cracks if left unattended. 1. Journal the risks and dependencies in a centralised repository. 2.Review the risks/ dependencies in the daily scrum of scrums. 3. Impacted teams should proactively set up working groups to address complex dependencies. By assigning user stories/ tasks to the relevant teams, the day to day tracking would become easier. 4. Review the risk/ dependencies during the retrospective sessions.

As I mentioned in my earlier comment: reduce dependencies amongst PBIs as much as possible. These put a big constraint on the ordering of the Product Backlog - which should focus on delivering value to users, not on dependencies. Risks are about uncertainties. What to do with uncertainties is to validate or invalidate them first. A practice for this is to write hypothesis statements and experiments to do this. Breaking down differently feels the best strategy for this.

"You" don't need to inform your team, PO, SM or others. Why, because identifying and handling risks is a team effort. The mentioned people should already be involved. For those sessions where not everybody is present, this informing, is done through the Product Backlog - because the Product Backlog is expected to be transparent (and as such understandable by all stakeholders), having a good look at it and as needed ask a colleague should do the trick.

When it comes to tools, the same tools as used in the different events should be used. There is no need to add complexity to this. Example.
