We are a leading integrated community provider in South East London, delivering a wide range of services to practices, Primary Care Networks (PCNs), and patients. Founded in 2016 with the mission to enhance primary and community care, OHL has been pivotal in offering comprehensive services to local communities, and supporting practices with large-scale solutions. We take pride in our dynamic, innovative team, which works flexibly with our System partners to deliver services at scale to benefit patients within the Integrated Care System. Our priorities for 2022-2027 are to support practices and PCNs, foster connections across London, deliver community-based services, and ensure long-term sustainability.

She’ll be discussing how Virtual Wards are shaping the NHS and their benefits to patient outcomes. We’re incredibly proud of our award-nominated Virtual Ward Lewisham NHS@Home service, which has been making a significant impact in patient care and outcomes. This service allows us to bring high-quality, remote care to patients, supporting their recovery and management from the comfort of their homes.
