Once one understands the reasons, it will be easier to overcome the issue. For example, if the size of the group is the reason, breaking the group into smaller discussion groups might be the solution. If it is a cultural issue, make adjustments to accommodate the cultural dynamics, and if it is because some members are overwhelmed by their seniors, be particularly accommodating of the junior members of the team to encourage participation perhaps by letting them drive the conversations.

Setting up clear objective and agenda for the meeting/discussions, expected roles by each participant is very important. Sometimes there are people who are submissive but having great ideas/solutions, however they get overpowered by aggressive team members. It is important as a leader to observe and encourage participation, if require by intervening, asking targeted questions and let the submissive person speak his mind out. Appreciate the great ideas for generating confidence.
