The HeartLife Foundation is Canada’s first – and only – national patient-led heart failure organization. We are a Federal Charity aimed at raising public awareness of heart failure, engaging patients, families, and caregivers to provide education and support, facilitate access to the latest research, innovations, and treatments, and advocate better care for all. Our mission is to transform the quality of life for people living with heart failure by engaging, educating, and empowering a global community to create lasting solutions and build healthier lives. Heart failure, a condition affecting over 750,000 Canadians, not only impacts lives but also carries a significant economic burden. Senator Martin recently highlighted The HeartLife Foundation's National Policy Framework during a Senate meeting, urging for: ● A patient-centered national standard of care: Ensuring all Canadians living with heart failure receive the support, treatment, and dignity they deserve. ● Bridging gaps in the healthcare system: Addressing province-to-province disparities in access to testing, care, and understanding of patient needs. ● Cost-saving measures: Implementing the Framework's recommendations, including early detection and multidisciplinary care, could generate substantial savings through reduced hospital readmissions and improved patient outcomes. HeartLife's Framework, developed in collaboration with esteemed partners, emphasizes: ● Early detection: Identifying heart failure at its earliest stages for timely intervention. ● Multidisciplinary care: Encompassing various healthcare professionals working together to provide holistic patient care. ● Continuous evaluation: Regularly monitoring and adjusting treatment plans based on individual needs. This patient-driven approach not only promises improved health outcomes but also offers significant economic benefits. International studies demonstrate that: ● Remote monitoring and multidisciplinary care can reduce heart failure-related hospital readmissions by 89% and costs by 63%. ● Improved access to specific testing could yield additional annual savings of millions of dollars.
