About us
CTN supports over 29,000 children and youth with disabilities and developmental needs. Through 110,000+ service visits per year, we provide intake, service navigation and coordinated service planning, assessment and diagnostic services, specialized clinics and rehabilitation services, including physiotherapy, occupational therapy and speech language therapy. Our support reaches children and youth in their homes, communities and schools, from birth up until their 19th birthday (or until completion of high school) through our network model of service delivery. Through the collective efforts of community partners, we provide and enhance services, champion system change and build capacity primarily in York Region and Simcoe County, as well as Central and West Toronto, Dufferin, Halton, Peel, Waterloo and Wellington. We also support families by connecting them with other community agencies who provide additional resources and programs outside of our partnerships. Our network model engages families at all levels within the organization, allowing us to provide coordinated services. CTN and the services provided by our network partners are funded by Ontario’s Ministry of Children, Community and Social Services (MCCSS) and are free for families. However, there may be costs associated with provision of special equipment, materials or supplies needed by some children.

CTN is looking for a permanent, full-time Decision Support Specialist who will collaborate and facilitate across teams to establish a vision for data-informed decision-making at CTN. This role is pivotal in enhancing data literacy among employees through experiential learning, ensuring data is not just a tool for validating hypotheses but a means to explore broader insights and opportunities. The Decision Support Specialist will leverage Business Intelligence tools—particularly MS PowerBI—to design, implement, and support data solutions. A strong background in statistical knowledge and a passion for mentoring colleagues with varying levels of data expertise are essential. The Decisions Support Specialist will be responsible to support clinical programs performance measurement activities through managing senior level data reports, participating in regular team meeting and will lead discussions around analytical requirements of the clinical teams. The Decision Support Specialist supports the implementation of the CTN data strategy, data quality activities and analytic products to support strategic decision making, quality, performance, and strategic initiatives at CTN. CTN is looking for a temporary, full-time (contract) Service Navigator who will be responsible for providing information and processing referrals for children, youth, and their families. The Service Navigator uses clinical judgement to review and prioritize referrals, complete strengths/needs-based assessment, link the client and family to appropriate services in the community and apply appropriate service delivery guidelines. The Service Navigator uses critical thinking skills to respond to and resolve issues, concerns, complaints, and risks.
