Implant Placement in Cases of Minimal Bone below the Maxillary Sinus Engaging the Lateral Nasal Wall When the bone below the maxillary sinus is insufficient, implants cannot be placed conventionally. In such cases, the implant can be anchored in the lateral nasal wall. Suitable implant choices are Basal Tornado or Basal Varioss. Engaging the Palatal Bone Another technique involves engaging the palatal bone by initiating the osteotomy laterally and targeting the palatal bone directly. The preferred implant for this technique is the Basal Classic due to its low-profile, single-thread design. This implant minimizes torque and is ideal for narrow, high-density bone to prevent fractures.

Accessory Implant Sites for Load Dispersion Key Points: ·        Accessory Sites: Lateral nasal wall, palatal bone, or mid-palatal engagement. ·        Purpose: o   Reduce the jumping distance between implants, especially in cases of sinus pneumatization. o   Prevent prosthesis fractures due to high inter-implant distance. ·        Note: o   These are load-dispersing, not load-bearing sites. o   Not suitable as standalone implants.

Basal Implants for Pterygoid Area Implant Choices: 1.      Tornado Implant: o   Smooth surface compressive implant. o   Allows perforation of the second cortex. o   Designed specifically for the pterygoid area. o   Requires only one implant. 2.      VarioS Implant: o   Requires two implants (double pterygoid technique). o   Not recommended as a single implant solution.

Design of Varioss Basal Implants 1. Thread Engagement ·        Cortical Threads: Engage the basal bone for anchorage. ·        Cancellous Threads: Engage cancellous bone through compression. 2. Passive Shaft Design ·        The shaft is thread-free and does not engage the first cortex. ·        Ensures the first cortex remains passive and unaffected. 3. Prevention of Peri-Implantitis ·        Avoiding threads in the first cortex minimizes the risk of resorption and peri-implantitis. ·        The smooth shaft design prevents thread exposure, enhancing long-term implant stability.

Features and Advantages of Varioss Implant 1. Dual Thread Design Cortical Threads: Located at the apex, these threads actively engage the nasal cortical floor. Compressive Threads: Positioned behind the cortical threads, designed to compress and stabilize in cancellous bone. 2.
