What does Lord Darzi have to say about pharmacy? In our continuing series of posts about the Darzi Report into the state of the NHS, we are today focusing on its specific observations about pharmacy. The NHS is a vast subject and Lord Darzi’s impressive report, produced in little time, had much ground to cover. However, we might have hoped that pharmacy would have rated more than 17 mentions in its 50,000 or so words. The focus is in the chapter ‘Access to community pharmacy’, in which the accessibility of community pharmacy is acknowledged as one of the NHS’s great strengths, noting that more than 93 per cent of patients living in areas of highest deprivation live within 1 mile of a pharmacy. However, pharmacies are closing in significant numbers (see the chart below, taken from the report), with around 1,200 pharmacies having shut since 2017. And even though pharmacies have greatly expanded the range of services offered, the total level of spending on the community pharmacy contract has fallen by 8 per cent. Darzi acknowledges the potential for community pharmacy to provide more value-added services for the NHS (such as the Pharmacy First programme) and also that nearly 30 per cent of existing pharmacists are independent prescribers. The report says that ‘There is huge potential for a step change in the clinical role of pharmacists within the NHS. Expanded community pharmacy services are likely to include greater treatment of common conditions and supporting active management of hypertension.’ But it goes on to note that ‘there is a very real risk that on current trajectory, community pharmacy will face similar access problems to general practice, with too few resources in the places where it is needed most.’ It is a start, acknowledging the potential and challenges facing the sector. We now await the 10-year plan, currently being drafted, to see in detail what the new government intends to do.
