Air Pellet Injury: Complex Surgical Case A rare and challenging case where a patient sustained an air pellet injury while playing with children. The pellet entered through the face and exited on the left cheek, severely injuring the parotid gland. The patient came to us 4 days later with intense pain, swelling, lymph node enlargement in the neck, redness, and clear signs of infection. A CT scan revealed the pellet lodged deep inside the superficial lobe of the parotid gland. We immediately took the patient to the OT for exploration and carefully planned the procedure. Using a C-arm for real-time imaging and ultrasonography for pinpoint accuracy, we were able to precisely localize the pellet without causing additional tissue damage. This advanced imaging allowed us to locate the pellet hidden in delicate tissue. Once we located the pellet, we successfully excised it, ensuring all damaged tissue was addressed. To guarantee a scarless closure, we utilized a 3-layer closure technique, which minimizes visible scarring and accelerates healing. This is why it is vital to limit the number of people in an OR and reduce air circulation & movement.
