The author of this document identifies a number of concerns that ring very true about decision making and lack of qualified supervision on construction sites. Sadly the role of the Resident Engineer is long gone, and all too often design information is indeed provided to Contractors who never refer to the Engineers again. Fundamentally this bridge is unusually designed: the Architectural concept being a cable-stayed bridge which would have slender light weight deck construction; being translated into a concrete truss with a tower and steel tubes for decoration purpose only was an immediate recipe for disaster. No doubt the decision to change from the Architect concept was driven by cost and a belief that by precasting a truss it could be installed in a single operation (probably with an overnight road closure). From that moment on the Engineer would have been under constant pressure to reduce member sizes to maintain the slender appearance of the bridge. However, the decision to post tension the diagonal strut, which is clearly a compression member that was cracked and undergoing compressive failure was in my option a decision of criminal incompetence. It is easy to see what went wrong in hindsight, but so often difficult to be the one who makes a stand against the wave of Parties who all think they know best and sadly only those directly involved will have learned the lessons. And since we're already talking depth of seal, lets look at why it is so difficult to create with diagonal openings. Within a solid tilt up concrete wall, depth of seal with proper annular space is not terribly difficult to create as you have the entire opening to work with. But say the pipe is a few degrees off of the cored opening and create point of contact or zero annular space? What does that look like? And what about gyp walls? How do we treat that?
