Allied health professionals play an invaluable role. Many show expert care,skills & empathy beyond that of doctors and specialists I have seen and/or worked with to date. (Knowledge means nothing without care & professional integrity) Regardless of the individual practitioner reflecting on the training parallels & therefore necesssary supervision; it’s important to emphasise the difference in supervision in the hospital setting (with tests/monitoring/supervision/huge teams to assign responsibility/ blame) Versus primary care (isolation/complexity in 10 min appointments/risks of undifferentiated presentations). The later requires Generalist training to risk stratify every ‘undifferentiated’ clinical presentation in the community. To avoid delayed diagnosis & treatment #clinicalacumen #tests #treatments needs to be enhanced in the community not siloed #integratedcare #cdc Modern undergrad/GP Training: Resuscitating newborns, caring for children with sepsis/lifethreatening asthma, medically & psychologically managing individuals with severe mental illness, working acute and chronic settings in general medicine, surgery , paediatrics, ent, obstetrics etc. Initially “supervised” day shifts & out of hours practice for clinical sign off. Reinforced decades of multiple undergrad/post grad exams testing the depth and breadth of all specialities and public health in order to practice independently. With expectation of continual professional development. Chronic disease management has become more not less complex as we live longer with new therapies and guidelines in 6 monthly reviews. If practices and government wish primary care delivery to focus on the number of #encounters and not quality/ safety … then it doesn’t matter who delivers it. If so, this must be safety netted with symptom #pathways & #diagnostics early in presentations. Not just addressing #access but addressing the unmet need for #accurate diagnosis(es), treatment & enhancing #expertise in the community We need to agree the values& expectations of our healthcare system, to then design the delivery of care to match a mixed workforce-not undermine it.
