In Canada, living with Inflammatory Bowel Disease (IBD) means facing not only physical challenges but also significant mental health burdens. Currently, approximately 300,000 Canadians are affected by IBD, a number expected to rise to around 470,000 by 2035. A recent survey commissioned by Takeda highlights the urgent need for enhanced mental health support for this population. The findings from Takeda’s “300,000 Reasons Why” campaign reveal that two-thirds of Canadians living with IBD experience mental health struggles linked to their condition. The survey found that 90% of respondents reported that IBD negatively impacts their productivity at work or school, with many feeling socially isolated due to their illness. Patient Carla Bertelli from Calgary shared her experience, detailing the mental toll of IBD and the frustration with insufficient support from healthcare providers. **** Exciting News! **** I am thrilled to announce the publication of our latest research on improving mental health for patients with chronic physical conditions. This study protocol outlines a cluster-randomised controlled trial embedded in Danish general practice. Target Group: Patients with type 2 diabetes and/or chronic ischemic heart disease experiencing poor mental well-being. Intervention: Problem-solving therapy delivered by GPs or general practice staff. Primary Outcome: Reduction in depressive symptoms after 6 and 12 months. Secondary Outcomes: Changes in mental well-being, anxiety symptoms, diabetes distress, LDL, HbA1c. Process Outcomes: Measures of implementation and mechanisms of impact. Patients are at the heart of what we do, and whenever I get to witness the moment a chronic pain patient finds relief for the first time in months or even years, it's such a powerful and rewarding reminder of this. September is Pain Awareness Month – a time to shine a light on a health issue that remains underdiagnosed and undertreated. Our recent survey of 3,000 chronic pain patients shows that the impact on the mental and emotional health of those affected is tremendous: it can impact sleep, relationships, lead to patients experiencing depression and anxiety. In addition, it is particularly young people, who should be entering their prime years, who feel this side of chronic pain the most. Weight gain is a side effect of antidepressants. Many people suffering chronic and persistent pain are prescribed these drugs and suffer the consequences. Bearing this in mind and that many who suffer also experience poor cardiometabolic health, the authors make the following recommendations: * assessing risk factors for obesity before treatment * monitoring metabolic health at baseline and regularly during follow-up * offering lifestyle interventions including regular exercise and healthy diet based on patient preference to optimize motivation * considering first-line psychotherapy for mild-moderate depression and anxiety disorders * choosing medications based on medications’ and patient’s weight gain risk * choosing medications based on acute vs long-term treatment * using effective, tolerated medications * switching to less weight-inducing antipsychotics/antidepressants where possible * using early weight gain as a predictor of further weight gain to inform the timing of intervention/switch options * considering adding metformin or glucagon-like peptide-1 receptor agonists, or topiramate (second-line due to potential adverse cognitive effects) to antipsychotics, or aripiprazole to clozapine or olanzapine. r.s. IMO — the most practical and impacting guidance will be positive lifestyle changes because they are immediately available, they shape future experiences, are low cost, low tech, achievable, possible and make up the bulk of the person’s time. Medication can be helpful, sometimes essential, yet skills to live well are always important. Who can help guide the person? A Pain or Health Coach. * clarify the person’s aims * create a contract based on who is responsible for what — patient (person) and practitioner * commitment — the person must decide if they are willing to commit to this path.
