Why Rising Risk Matters in Population Health Management 💡 Patients in the rising-risk category have gradually worsening health, indicating an increasing likelihood of developing new or additional chronic conditions or experiencing adverse health events in the near future. Targeted interventions in this population are crucial to determining better health outcomes for patients and mitigating health care costs. In our latest whitepaper, you’ll learn: ✔️ How to segment populations based on health needs ✔️ The critical role of stratifying risk for optimal interventions ✔️ Leveraging ACG® System analytics to identify and manage rising-risk cohorts ✔️ Practical steps to improve health outcomes and reduce costs Download our whitepaper today to learn how the ACG® System can help identify rising-risk patients for early intervention: https://lnkd.in/ekwHE7zh

Americas Health Rankings Senior Report This week, the United Health Foundation released the 12th edition of the America’s Health Rankings® 2024 Senior Report. As an OB-GYN, I am involved with taking care of women throughout all stages of their lives. As a Chief Medical Officer for UnitedHealthcare, I now have the perspective of helping thousands of patients through a population health lens. As Americans live longer and the population ages, we want to help them thrive and fully participate in their communities. Here are a few highlights from the 2024 Senior Report, revealing both progress and ongoing challenges: The number of geriatric clinicians increased 4% between 2022 and 2023. Early deaths significantly decreased 8% between 2021 and 2022; however, the rate of early death is still higher than the pre-pandemic rate. The poverty rate increased 6% nationally from 10.3% to 10.9% between 2021 and 2022. The number of seniors experiencing food insecurity increased 8% between 2020 and 2021 — the first increase among older adults since 2014. Whether we are treating a single patient or implementing community programs, we need access to reliable data about the things that affect our communities' well-being to address those upstream factors. That is the type of resource America’s Health Rankings wants to be to our nation, now and into the future to help improve public health. I hope you’ll take a moment to check out the 2024 Senior Report and use it as an essential tool for public health improvement for our senior population. https://lnkd.in/gPmH6Aif

More than half of the world’s population takes at least one drug each day, and the demand for pharmaceuticals is only expected to increase as the global disease burden continues to grow 1. The key reasons for poor adherence are patient forgetfulness, anxiety about treatment-associated adverse effects, low motivation due to a perceived lack of efficacy, poor health literacy and aversion to the health belief model, and stigmatization 2,3. Improving adherence is recognized as one of the most impactful and cost-effective strategies for improving the health of the general population, yet it has not garnered the same attention as other approaches for improving wellness 2. #smokingcessation #treatmentsuccess 1. 2/100 🌍 Understanding Global Health Metrics: A Tool for Better Decision-Making 🌍 In an interconnected world, data-driven insights have never been more critical to public health and policy. Global health metrics provide a structured view of health outcomes across populations, helping us address challenges with precision and equity. Metrics like the Global Burden of Disease (GBD) study measure years of life lost (YLLs) and years lived with disability (YLDs), allowing us to quantify and prioritize health conditions by impact. Through these insights, we identify trends in disease prevalence, mortality, and the effectiveness of health interventions across different regions. Data like this helps policymakers, healthcare providers, and advocates make informed decisions, ensuring resources are targeted to areas of greatest need and paving the way for impactful policies on the ground. At a time when global health challenges are shifting rapidly, reliable metrics are indispensable in our pursuit of a healthier, more just world. Let’s keep pushing for data that leads to meaningful change! #GlobalHealth #HealthMetrics #GBD #PublicHealth #HealthPolicy #SocialJustice

Health System Performance Measurement Framework provides a system-level overview of the key dimensions of performance and contextual elements that must be studied when assessing the achievement of health system goals for individuals and for the general population. It allows for the assessment of the collective contribution of all parts of a health system, as well as how each of the system’s distinct components (such as hospitals or long-term care facilities) connect to overall system performance. The framework provides a foundation for selecting measures or indicators that you can use to monitor health system performance
