One of the hottest topics in #sportscardiology is whether or not #ECG screening should be part of a pre-participation physical exam for young athletes. In this Healio At Issue feature, we got some prominent sports cardiologists to weigh in. They said: 🎤 David Shipon: "The notion that ECG screening should not be done in athletes is antiquated. Effective guideline-based screening should be done on a wide scale." 🎤 Eli Friedman: "The question organizations must now answer is whether their ECG screening program is up to the contemporary standard of screening to identify and risk-stratify. If the answer is “no,” then it is clear that the ECG should not be used." 🎤 J. Sawalla Guseh, II and Ryan Quinn: "It is essential for clinicians to fully understand the benefits and limitations of ECG screening and to apply it judiciously." Today, I had the pleasure of attending the Sports Related Concussion 2024 Update with Dr. Dean Padavan, part of Atlantic Health System’s Sports Medicine Webinar Series. The session explored concussions as traumatic brain injuries from forceful impacts, triggering a cascade of biochemical events that can alter brain function. Each year, 1.7 to 3 million sports-related concussions occur, with a high incidence in high school athletes—19% report at least one concussion per year, particularly in football, with female athletes having the highest rates. Dr. Padavan highlighted important signs of concussion, such as motor issues, disorientation, spontaneous head shaking, and stressed “No Go” indicators like ataxia and impact seizures that require immediate removal from play. Common symptoms like dizziness and memory issues are managed with tests like VOMS and a Sub-Symptom Exercise Protocol to allow for gradual, monitored recovery. This update reinforced the need for conservative management and thorough return-to-play protocols to prevent long-term consequences like CTE, safeguarding athletes’ health and future. #ConcussionAwareness #SportsMedicine

Associations between growth, maturation, and injury in youth athletes. Interesting new scoping review from Gemma Parry and colleagues recently published in British Journal of Sports Medicine. The review found that injury incidence and burden tend to increase with advancing maturity status. Specifically, injuries related to growth spurts, such as apophyseal and osteochondrosis injuries, peak during the adolescent growth phase (circa-peak height velocity). Exercise in Heat in a Patient with a History of MH.  One of the MHAUS consultants received a question concerning a young college student with a history of Malignant Hyperthermia (MH) in the past year who is looking forward to participating in his College’s football team. Should he be cleared to play? Given that a percentage of patients who experience heat related disability/heat stroke harbor a DNA variant causal for Malignant Hyperthermia, what advice should be given concerning becoming a football player undergoing rigorous practice? Should the DOD policy of denying military service if there is a history of MH be applied in this situation? What other organizations have a policy on exercise in heat with a personal or family of MH? Should dantrolene be used in treatment of heat stroke or similar disorder? As far as I can determine, there is no definitive statement on the issue except to use surface cooling in management of heat stroke but not dantrolene. What was advised in response to the question is verification of the MH episode and whole exome/whole genome sequencing and determining if there ever was a history of exercise induced muscle breakdown in the patient or family . For the interim, it was advised that the student not participate in the sport.  If you know of policies relating to this problem, please let me know. This is a big problem related in part to MH, but with further implications.

🚨New Publication Alert! We know maintaining PA beyond a structured exercise intervention setting continues to be challenging for older adults. We found compared to those who reported no barriers, young older adults who reported one or more barriers to PA maintenance were less active 10 yr following STRRIDE I. Additionally, we found how participants respond to a structured exercise intervention in certain clinical variables may be the most indicative of future PA maintenance.
