When our employees had the chance to meet in person with the people whose lives we touch – patients, caregivers and families – they realized something unexpected. What factors are associated with timely transition from pediatric to adult primary care? We used all-payer claims data from Vermont to look at this question and found: - Individual level: young men are substantially less likely to transition to adult primary care compared to young women - Family level: Patients are less likely to transition to adult primary care when another family member is also seeing a pediatrician (presumably a younger sibling). They are more likely to transition to adult primary care with another young adult in the family (presumably an older sibling). - Provider level: Pediatricians with higher patient turnover are more likely to have patients transition to adult primary care. - County level: Robust capacity to provide primary care to children and limited capacity to provide primary care to adults are both associated with non-transition to adult primary care. We also did some fun network analysis to show that we could identify communities of providers associated with high and low rates of transition to adult primary care. A Sunday shoutout to fellow caregivers. Medication 💊 management is critical. While drug costs for Medicare recipients are lower, remember when budgeting that once your loved one transitions from independent living to Assisted Living, that monthly cost category jumps 900%! Why you ask? In the AL setting medications are shipped in from LTC pharmacies. Great people and companies. However the cost to do that is passed straight to the family. For example, while my Mom was in AL for 8 months, her monthly medication costs were $1,,200. When she transitioned back to independent living, we took over medication management. Thanks to the good folks at GoodRx, her monthly medication cost dropped to $120. #caregivers #lovemoms

Something to consider for #vbc: The average Medicare beneficiary sees two specialists, and 30% of beneficiaries see five or more(!). Managing VBC contracts with multiple providers and overlapping episodes can be complex (think a pregnant woman with diabetes, a diabetic/CHF patient undergoing hip replacement). Some helpful ideas in the blog here (e.g. the importance of provider transparency), but of course, it's all about the specifics of your situation.
