At-risk behaviour. Totally unacceptable. The question is what triggered this behaviour? What was the reason for personnel to go into the excavation/ hole? Looks like a ventilation raisebore. Are there systems guiding on how to exit such excavations? There should be specifically designed equipment/ lifts to lower and lift personnel from these areas. Food for thought.
This is not acceptable! For me the project manager here would be sent home immediately.

Such horseplay at sites can be the last game of life one plays. It is the responsibility of anyone around to STOP such acts and make the systemic changes needed for assurance. ROKO TOKO or Don’t Walk Past is an important tool to practice. Think of a project manager as the person at the center of the project, making sure everything runs smoothly. They start by identifying what the project should accomplish, then break it down into smaller tasks and milestones. Throughout the project, they keep track of progress, ensuring that each part of the project stays within budget, meets deadlines, and reaches the quality standards as required.
