In this role you will be part of a dynamic and diverse integrated team of clinicians providing services for adults and children. You will have the opportunity to triage clients both via phone and in person, to determine the next level of care. In the clinic, you will be administering injections and ensuring that care plans are followed. You will also provide care coordination and resource utilization to the clients you serve. “Do you feel safe at home?” “Are you experiencing any violence at home?” “Have you had thoughts of harming yourself?” I recently found myself in the Emergency Department and while I am normally in the nurse role, this time I was the patient. The triage nurse asked me the questions above in rapid fire succession without waiting for me to answer any of them. The door to the triage room was open, so everyone in the waiting room could hear our dialogue and my answers. I had a friend with me who was not asked to leave the room. The triage nurse never looked up from his computer to look at my face, body language, or response. He just assumed I would say “I’m fine” and move on, like most patients do. Screening for violence, suicidality, trafficking or any of the above is an absolute waste of time when it’s not done correctly. I would guess the exhausted, overworked triage nurse probably has never been trained on screening and is checking a box and moving onto the next thing. But don’t we want screening to work? Isn’t the point to identify patients who need help? Screening can be highly effective. I have witnessed it many times. But it requires healthcare workers to understand how to create felt safety, build rapport, and help patients feel valued and seen in a very short amount of time. I helped a forensic program in Alberta implement a screening process last year and 71% of the patients they screened, screened positive for some concern related to exploitation or trafficking. The nurses have done an amazing job of making their patients feel safe and seen before they dive into a questionnaire. Screening works well, when it’s done right. Most of the time, it’s done abysmally, and patients won’t disclose. Thurs morning reflection I had a visit to a community hospital this week. As I was viewing the day care and rehab department, I was making observations and commented on the utilization of speech therapy and how it enhanced the recovery of stroke patients. The head of department for care services asked "Are you trained in physical therapy or in occupational therapy?". He was surprised that I was asking certain questions about rehab time and the 'technical terms' I was using. I replied that I was not and I was a social worker by training. Due to my years in the hospital setting. I explained that I was given the exposure of working cases using a team approach, and thus my lens towards patient care is multi-dimensional. Trans-Disciplinary is the new buzzword we must all take note of. As professionals in the eldercare setting, we have to be more person-centric in our understanding of the needs, and which professional can enhance the care and recovery more effectively. Its a humble reminder to me that no one professional holds the absolute power in helping our seniors to age in place and live a life of fulfilment in the community.
