I have respect for ISMP. Yes...counterfeits exist and YES there are those that will certainly try to not follow the rules. However, ISMP is stating here to NOT use compounded versions of Tirzepetide of course siting the 2012 NECC issue..THE worst possible case scenario. It also INCORRECTLY states that the drug is ONLY available through Eli Lilly. YES the official drug as marketed is only available through them. However, an Active Pharmaceutical Ingredient can be used through a licensed supplier for the powder to be compounded. It's also worth noting..compounding is being done every day. Go tell a Urologist that it's no longer safe to order TRIMIX for his ED patients. Perhaps compounded Opthalmic meds should no longer be done. The argument NEEDS to stand across the board...if your argument is that compounding can lead to these health risks...then the risk exist for all and not 1 drug. That means...ALL COMPOUNDED IV ANTIBIOTICS...ALL IV INOTROPES..ALL COMPOUNDS FOR PETS..EVERYTHING. Yes the issue is real for Tirzepatide, but let's focus on WHY Eli Lilly is so focused on this...MONEY..just as much and the counterfeiters. In my career I have never seen a manufacturer test a compounded drug to prove a point. Plus, if we are comparing a manufacturer ability to produce vs a compound, the 2 will NEVER be equal.

🌟 Tria Triumph: GLP-1’s are Not for Everyone! 🌟 A patient with diabetes and a history of a heart attack initially faced GI discomfort from metformin IR. After a change in medication to Ozempic, their GI symptoms worsened dramatically. The patient described the issues as “wicked” with severe gas, nausea, and diarrhea. Our Tria Health pharmacist stepped in, thoroughly investigating the timeline of symptoms and medication changes. By pinpointing Ozempic as the likely cause, the pharmacist recommended discontinuing it and returning to metformin, this time in the extended-release (ER) formulation. Today, the patient’s HgbA1C and blood glucose levels are at goal, thanks to careful medication management and personalized care. In recent years, GLP-1 receptor agonists have become vital tools for managing type 2 diabetes and obesity. Their growing popularity, however, has led to increased demand, product shortages, and the rise of compounded alternatives. While compounded GLP-1 formulations may appear to offer affordable solutions, they come with serious risks, including concerns around dosing accuracy, potential counterfeit products, and lack of FDA regulation. As highlighted in the American Diabetes Association's recent statement, compounded versions bypass critical safety measures—posing dangers for patients who are not closely monitored by healthcare professionals. We welcome the ADA’s guidance against unapproved compounded medicines, which rightly points out the “uncertainty about their content, safety, quality, and effectiveness.” No patient should be exposed to unapproved knockoff drugs with heightened risks, particularly when safe and effective FDA-approved medicines are available. Mass-produced unapproved drugs may expose patients to potentially life-threatening risks. Patient safety remains Lilly's top concern, but our efforts alone are not enough. We appreciate the ADA’s continued commitment to patient safety.
