Did you know that your skin, the largest organ in the body, is rich in cannabinoid receptors? These receptors are crucial for maintaining a healthy skin tone through the endocannabinoid system. A growing body of research suggests that cannabinoids hold significant promise in treating various skin conditions, including eczema, psoriasis, acne, pruritus, hair disorders, and even skin cancer. The science behind this is compelling. Cannabinoids interact with the endocannabinoid system in ways that can help modulate skin functions, reduce inflammation, regulate oil production, and support the skin's natural healing processes. This emerging research highlights the potential of cannabinoids as a natural and practical approach to skin health. Studies such as those by Tóth et al. (2019) and Oláh et al. (2014) have shown that cannabinoids can reduce the symptoms of psoriasis and acne, respectively. Moreover, research published by Ramot et al. (2013) indicates the potential for cannabinoids in treating hair disorders. The therapeutic potential of cannabinoids in dermatology is becoming increasingly recognized in the scientific community. Dermatology is frequently perceived as a minor, superficial discipline, but the reality is quite the opposite! Skin diseases are a highly relevant cause of health loss globally in terms of: 📈 Prevalence: One third of the population suffers from at least one skin condition – the number of new cases per year of psoriasis, atopic dermatitis and melanoma have doubled during the past decades! 📌 Burden of disease: The 15 most common and researched skin conditions combined are the fourth leading cause of non-fatal disease burden at the global level. 🛏 Quality of life: Chronic skin diseases impair patients' quality of life more drastically than asthma, epilepsy or diabetes! 💰 Costs: Skin diseases in the US incur direct healthcare expenses of $75 billion, with an additional $11 billion in lost opportunity costs. Given that these figures are from 2013, current estimates suggest even higher costs today. 🩺 Follow-Up Post: Adapting Acne Treatment for Better Outcomes 🔄 Adjusting the Approach: A Journey in Acne Management In the ever-evolving world of clinical dermatology, one key lesson stands out: flexibility is critical. I recently shared a case of a patient struggling with moderate-to-severe inflammatory acne that was compounded by lifestyle factors like stress and sleep. Despite his sleep situation mild improving and starting treatment with fusidic acid and azelaic acid, the progress over 1 weeks was suboptimal and not satisfactory. This prompted me to reassess and refine the treatment plan. 💡 The New Approach After careful evaluation, I shifted to a regimen designed to tackle both the inflammatory and bacterial components more aggressively: 1️⃣ Erythromycin Lotion (Topical Antibiotic): Targets Cutibacterium acnes, the bacteria contributing to acne inflammation. 2️⃣ Adapalene (Topical Retinoid): Acts as a comedolytic and anti-inflammatory to prevent new lesions and reduce existing ones. 3️⃣ Doxycycline (Oral Antibiotic): Provides systemic anti-inflammatory and antibacterial effects, ideal for moderate-to-severe acne. 🧴 Supportive Measures: To minimize side effects and improve adherence, I emphasized: Moisturization: To combat dryness from adapalene. 📊 The Plan Ahead Acne management requires patience and precision. While I expect significant improvement within 4–8 weeks, the key is consistent follow-up to track progress, adjust therapy as needed, and address any concerns. 🔬 This case reinforces the importance of balancing clinical experience with evidence-based decisions. Treatment isn’t one-size-fits-all—what works for one patient may need adjustment for another. 📝 Takeaway: In dermatology, flexibility, patient communication, and a holistic approach are essential for successful outcomes. Great to see Johnson & Johnson embracing #diversity in advertising #tremfya, as there is a high prevalence of #psoriasis in black and hispanic communities, yet 93% of people in #psoriasis commercials are white. What we now need to see is diversity in trials, as only 14% of participants in phase 3 studies are people of color, and fewer than 20% of images in dermatology textbooks show #psoriasis on darker skin. Johnson & Johnson has taken steps against underrepresentation, having launched a study in 2022 dedicated to people of color living with #plaquepsoriasis and scalp #psoriasis, with the sole intent to address gaps in representation.
