Utilization Management and Latest Reviews Utilization management (UM) programs were put in place to facilitate clinically appropriate #patienttreatment plans and cost effective care. UM is used prior to pre-authorization during concurrent review and after a patient is discharged from a hospital or other clinical setting. As a result of the information gathered in UM, connections to disease management, care management, and population health program offerings can be linked to UM programs with collaboration between #providers, #payers and #patients. As reported by Medecision, less than 32% of health plan leaders rate their UM tools as “very effective” at identifying health events in time to have a positive impact. An estimated 25% of health plan leaders said the UM tools are “easy to use”. This data reflects that some regard UM programs as fragmented and challenging. Social determinants of health (#SDoH) are the non-medical factors that influence 30–55% of #healthoutcomes and affect access to #healthcare. They include the conditions in which people are born, grow, work, live, and age, and the wider set of factors shaping the conditions of daily life. These wider factors include economic policies and systems, development agendas, social norms, social policies, and political systems. To ensure that health systems focus on SDoH, #healthleaders should embed a consistent SDOH approach to standardization, collection, analysis, and dissemination of #data across the different care delivery models. There is a need to advance evaluation and build evidence for strategies that address SDOH to reduce disparities and promote #healthequity. It is necessary to establish criteria, actionable steps, and strategies for partnerships, collaborations, and relationships that improve #healthoutcomes over the long term. Community engagement is pivotal to fostering meaningful, sustained #communityengagement across all planning and implementation phases. Stakeholders should invest in required #infrastructure and capacity building to strengthen and sustain infrastructure such as workforce, training, and access to financial resources required to address SDOH and reduce #healthdisparities. It is critical to identify evidence, tools, and resources to enhance communication about policies that affect SDOH with #policymakers and other #stakeholders. The hammer down from overutilization focus is a nail in the coffin of patient engagement. Observations of primary care over 40 years reveal the truth about the way that health care designers defeat necessary patient engagement. There is no excuse for massive deficits across primary care, mental health, women's health, and geriatrics The United States is unique in so preventing patient engagement despite sufficient finances. About 40% of the nation in 2621 counties lowest in health care workforce has half enough of each basic health access specialty and even more compromises in the delivery team members. Where the population is most complex and most vulnerable, one on one person to person innovation is most required and is most prevented. Discussions of patient engagement are pointless with so many suffering from underutilization and inappropriate utilization. You cannot fine tune a catastrophic design.
