Diffuse Diabetic Macular Edema: A Fourth Pathogenesis is presented. Based on two newly-recognized pathogeneses, it seems confirmed that all DDME eyes treated for months/years by medications and/or grid laser are tractional. These pathogeneses are undetectable by the standard OCT imaging: 1) 3D-OCT for extrafoveal traction and, 2) transitional-phase type (previously termed 'vasogenic'); it is detectable only ultra-structurally. These pathogeneses explain and support the 92-100% achievement of durable macular drying by early PPV in naive-treated eyes. These also explain the failures in achieving durable macular drying by any of the current medications, including those under studies using various approaches. Therefore there is no need (for extrafoveal traction) or possibility (for transitional-phase type) to detect either of them prior to surgery. PPV, and grid laser in specific circumstances, are also associated with the lowest burden and cost. These and other insights are presented in my current Editorial article. Definitely need more exploration so we can all learn from the edge female doctors may bring. Ironically, patients often think my male medical student is the doctor and I’m the nurse. According to a new study led by Dr. Julie Hallet, scientist in the Odette Cancer Research Program at Sunnybrook Research Institute, care in hospitals with greater than 35% female anesthesiologist and surgeon teams was associated with a reduction in the odds of postoperative complications. “These results are the start of an important shift in understanding the way in which diversity contributes to better quality care around the time of surgery,” says Dr. Hallet. Thought provoking .... nicely written article from a patient's point of view. So much to say for genuine patient centred care! Things are not always black and white ..... and when they're not ..... we need to listen to the bigger picture. I found this refreshing and a reminder. We can all get so dogmatic in our care and sometimes "evidence based medicine", forgetting the original proviso that ALWAYS said the patients needs, wants and values are part of the process, can lead us to be more dogmatic than ever!
