At the International Liver Transplantation Society, Dr. Janina C. Eden shared UMCG's experience with prolonging machine perfusion for up to 20 hours using cold machine perfusion. Their study showed that donor livers can now be stored longer, making it possible to perform transplants during regular daytime hours. This breakthrough has proven to be just as safe and effective as immediate transplants, with no increased risk of complications. Since early 2023, almost all liver transplants at UMCG have been scheduled during the day, improving team readiness and overall outcomes. Instead of flushing a donor liver or heart of liquids and cooling it in a cold preservation solution, which is the traditional method, the organ is perfused with warm, oxygen-infused blood via a system of pumps, which sustains the organ and its function in a life-like state. This enables the organ to remain viable for transplant for a longer period of time, and to be transported longer distances. An organ donated after circulatory death can be optimized for transplant using new technology such as ex-vivo machine perfusion or with normothermic regional perfusion. Using these technologies also increases organ availability for transplants. “This fundamentally changes organ preservation,” says surgical director of liver transplantation at Mayo Clinic in Arizona. “It leads to functional improvement without the obligatory organ injury that occurs with donor death and the classic organ recovery process,” he says. Super proud that we’ve already adopted this model and are transplanting DCD hearts via normothermic regional perfusion.
