We're committed to promoting Healthy Ageing in partnership with our community. In the 2023-2024 financial year we supported 64 residential aged care homes and launched the Healthy Western Sydney service directory. Our Urgent Care initiatives have also provided essential care to 15,105 patients with non-life-threatening conditions, helping to alleviate pressure on local emergency departments. Additionally, we’ve supported MyMedicare registration, with an impressive 88% of eligible practices in Western Sydney now registered. Together, we’re strengthening relationships between patients and practitioners for a healthier future! All too often, care decisions are made during a health crisis, leaving older people unable to fully participate due to cognitive decline or urgent medical needs. This can place immense pressure on families, leading to rushed choices that may not reflect what their loved ones truly want. At the Live-In Care Hub, we’re dedicated to giving families the tools, resources, and support they need to plan ahead. Early discussions about care options allow families to: 💬 Help older loved ones express their wishes 🤝 Make informed, thoughtful decisions together 🧠 Avoid the stress of last-minute arrangements By normalising early planning, we can ensure that older people retain their voice in decisions about their care and feel valued and respected throughout the process. Let’s start the conversation today—because everyone deserves the opportunity to live their later years in comfort, dignity, and familiar surroundings.
