My Friday good news celebrates the heroes of value-based care: primary care physicians and teams. The most successful #Medicare ACOs, those delivering value for Medicare and Medicare beneficiaries and sharing in that value, are primary care-centric #ACOs. We know how they do it; the playbook often includes support from enablers like Aledade, Inc.. Dr. Menard and his colleagues didn't give up when success wasn't immediate. How do we grow more primary care-centric ACOs and generate more value for Medicare and more access and quality care for beneficiaries? There are some health system ACOs investing in primary care and integrated teams. But it might take more forceful nudges from Medicare to get big health systems participating in total cost-of-care risk models. Or, we might need payment policies to give primary care more independence to practice comprehensive care. We recently spoke with Dr. Sandeep Yadavalli, MD, FACP, FHM, Associate Chief Medical Officer for Value-Based Care with Sound’s Long-Term Care Management accountable care organization, to learn what "bringing better to the bedside" means to him. For Dr. Yadavalli, it’s about going above and beyond the standard of care. “We all come into clinical care to do the best we can for our patients,” he shared. “But how do we reach them, not just as patients, but as people?” Dr. Yadavalli believes that when we treat patients as individuals, understanding how their care fits uniquely within their lives, we’re able to impact them in ways that go beyond measure. Previous studies in this area seem to point to a very poor application of evidence-based guidelines in clinical practice, is it improving? It’s important for clinicians to empower patients to understand the WHY behind the chosen intervention …and that implies the clinicians have to be able to explain what the science says. How often do you mention evidence in your clinical practice? How many interventions are you performing just because that’s what PTs have done for decades? Every single intervention on every single patient deserves critical thinking and application of science, blended with clinical experience.
