SUD is fundamentally a brain disorder. The region of the brain that mediates reward seeking behaviors, the meso-limbic cortex, functions differently in those with SUD — for reasons not yet fully understood — as compared to those who don’t present with SUD. When people are in active addiction, the decision to misuse substances is not a conscious one. While this may seem incongruous since SUD starts with a volitional choice, the area of the brain responsible for executive decision-making (by definition volitional choices), the prefrontal cortex, is overridden by signals from the meso-limbic cortex. The key to sustainable and long-term recovery is to stop misusing substances long enough so the brain’s neural pathways can reset and reform to enable the prefrontal cortex to reassume its executive decision-making role. This seems tricky since making a decision to seek treatment is a volitional one. But it’s instructive to remember that the vast majority of people with SUD DO want to stop misusing substances but despite willpower, the urging and threats from loved ones, friends and others, and often dire consequences, find it extremely difficult to stop. Inpatient treatment can be very helpful. But the real work begins when someone with SUD returns to their old environment after treatment. This is where ARCNH comes in. Our recovery coaches help participants (those with SUD) begin to develop a long-term recovery plan to enable sustainable and durable recovery and support them in their efforts to succeed in doing so. The really good news is that we know from peer-reviewed articles and longitudinal studies that if people with SUD can maintain continuous sobriety (or mainly continuous sobriety meaning no more than a few slips without long-term returns to misusing substances) for a full year, they have an excellent chance (>90%) of establishing long-term and durable recovery. SUD is a brain disease. Long-term recovery IS possible. As difficult as it may be to do so, please remember that no one I’ve yet met wanted to become addicted to alcohol and/or other drugs. Their continued misuse of substances has nothing to do with intelligence, willpower or lack of courage. Keeping this in mind will help everyone with SUD as well as those who love and care about them since stigma and shame only serve to reinforce maladaptive behaviors and the continued misuse of substances.
