ATTENTION MCH & Public Health Connections: March of Dimes 2024 "Nowhere to Go: Maternity Care Deserts Across the US" is now available online! I relied on this report to shape my academic research and public health courses. Today, I am thrilled to guide the incredible March of Dimes team that produced this powerful document. I am proud of our work and the solutions we propose to address the challenges facing maternity care in the US. Key Highlights 💜 Since our 2022 national report, approximately 2% of US counties became maternity care deserts due to the loss of birthing facilities or obstetric physicians. 💜 More than 100 hospitals (1 in 25) have closed their obstetric units, leading to over half of US counties lacking a hospital with an obstetric unit. 💜 The most recent data from 2022 shows that 85% of births were attended by an OB-GYN, who make up more than 70% of the obstetric workforce. However, to supplement the declining OB-GYN workforce and improve mom and baby health outcomes, integrating Certified Nurse Midwives (CNMs) and Certified Midwives (CMs) into the health system and training family physicians in maternity care is essential. MemorialCare’s Saddleback Medical Center, Orange Coast Medical Center, and Miller Children’s & Women’s Hospital have once again been recognized as High Performing in Maternity Care by U.S. News & World Report, which is the highest award a hospital can earn in maternity care. This year, U.S. News added a new measurement focusing on transparency in racial/ethnic disparities. The Neonatal Care (Leave and Pay) Act 2023 is expected to take effect in April 2025, giving eligible parents up to 12 weeks of paid leave to care for their baby if neonatal care is needed shortly after birth. When Hungarian doctor Ignaz Semmelweis started working at Vienna General Hospital's maternity clinic in 1846, it's unlikely he expected to find himself face-to-face with the perfect set-up for a natural experiment. Yet that's exactly what he found. The clinic was divided into two wards - one staffed by doctors and medical students, and the other by midwives - with the maternal mortality rate almost five times higher in the doctors' ward. By comparing the conditions between the two wards and testing a series of hypotheses, Semmelweis was ultimately able to identify the true cause of the mortality difference - the doctors were performing autopsies before delivering babies while the midwives weren't. And this led to the discovery of the importance of hand washing in controlling the spread of disease. Here's the thing... A/B tests are the gold standard for causal analytics. But they're not always practical, or even ethical, to implement. Had Semmelweis randomly divided the patients in the hospital into two groups - those who would have their babies delivered by doctors or midwives who *did* observe hand hygiene and those whose babies would be delivered by doctors or midwives who *did not*. That would have been very wrong. However, by operationalising the randomness that existed in his environment, and using that to devise a series of natural experiments, Semmelweis was able to identify a causal relationship that we all still benefit from understanding today. Just because conducting an A/B test might not be possible doesn't mean causal analytics isn't something you can use. Natural experiments are all around us. You just need to know where to look. p.s. University Hospital has been named to Newsweek’s list of America’s Best Maternity Hospitals 2024. The award list was announced on May 8, 2024, and can be viewed on Newsweek.com. High-quality maternity care, provided from pregnancy through birth and postpartum is key to the long-term health of newborns and women who give birth. Given that maternity care is a major component of health care, Newsweek and Statista, a leading statistics portal and industry ranking provider, have partnered for the third time to identify America’s Best Maternity Hospitals in 2024. The ranking is based on three pillars: • Nationwide Online Survey: Health care professionals and hospital managers with knowledge about maternity processes, such as neonatal and perinatal doctors, nurses, and midwives, were asked to recommend leading maternity hospitals in the United States. • Patient Experience Survey: Survey of the overall patient experience in the hospital. • Hospital Quality Metrics: Indicators relevant to maternity care were considered. Maternity failings have been in headline news over the past couple of months, with inquiries opening on the care of mothers and babies across the UK. Issues with maternity care persist, which is why HIWM’s Maternity and Neonatal Safety Improvement Programme (MatNeoSIP) is so important. MatNeoSIP aims to reduce the national rate of preterm births from 8% to 6% and reduce the rate of stillbirths, neonatal death, and brain injuries occurring during, or soon after birth by 50% by 2025. Births can bring challenges for both mother and baby - that’s why HIWM is involved in the Maternity and Neonatal Safety Improvement Programme, led by NHS England 🍼 The programme aims to: 👉 Reduce the rates of maternal and neonatal deaths, stillbirths, and brain injuries that occur during or soon after birth by 50% by 2025. 👉 Set out safer maternity care, reducing the national rate of preterm births from 8% to 6%. 👉 Reduce unwarranted variation in maternal and neonatal care, providing a high-quality healthcare experience for all women, babies, and families. Certification excellence standards help organize the disease specific management program. This helps to maintain a consistently high level of quality, using effective data-driven performance improvement. To qualify for the AACI Clinical Excellence Certification, Maternity Services (Mother and Child Care) must operate either as an accredited freestanding facility or as a service within a larger healthcare organization with acceptable accreditation… At AACI we believe that every family using maternity services should get the best care possible regardless of their location.
