Let me present a piece of supporting evidence for Benjamin Schwartz, MD, MBA’s observations. At the ISTA 34th Congress last week I was sitting 1 row behind the Editor-in-Chief of the British Joint Journal, Dr Fares Haddad. The exact context of the discussion I can’t relate perfectly but the comment he uttered regarding papers/data was “90% is garbage”. I share a cynicism around the mighty p value and Big Data drivel but this frank admission shocked me. WOW. I have experienced being on both sides of the peer review process and reviewers are honest but often not experts or experts with blinders, limited to the status quo or statistical machinations. This may work for a Mayo type institutional database but people learn by sharing ideas and ideas are what changes the course of incremental progress. This exchange occurs in hallways, tables, and pubs at meetings. I often wonder about this term, “Value based Care”. From the surgeon perspective, it has undeniably translated to “Do more for less”, which is a bad strategy unless you’re a Pharoah too. From a facilities perspective it’s good as with fixed payments, lowering costs and increased volumes produces revenues. Thus, the surgeon ASC owner incentivized to churn and burn but hopefully not burnout. Finally, the insurers, who it really doesn’t matter, as they own the playing field and governmental agencies setting rates and policies entirely. They just move the pieces around, guaranteeing heinous profits at the expense of a citizenry that, to them, pays more to be ill than healthy. Value here would be to invest in wellness and have capped profits as a healthy, financially sound country, unhampered by bloated and denying for profit insurers, is a more productive country. Lots to consider But let’s thank the Journal of Orthopaedic Experience & Innovation for providing a forum for open discussion of topics not typically found in traditional publications. Hey, don’t these “traditional” publishers make outlandish profits on the backs of researchers too? It’s a Mad Mad World, isn’t it? Guess it’s always been somewhat unfair. #followthebod #Bodsquad

A differentiated approach to leg alignment with an individualized target range for medial and lateral cartilage defects, whether physiologic or by osteotomy, is indicated to optimize PRO after M-ACI. Patients who achieve these individualized ranges are significantly more likely to achieve PASS. They tend to benefit more from M-ACI and achieve better MOCART scores in the long-term. In order to provide sufficient mechanical stimulus for graft maturation but avoid mechanical overload, orthopaedic surgeons should be aware of the newly identified ranges of leg alignment to ensure optimal outcomes for their patients. Survey of 205 orthopaedic surgeons revealed 73% either always return explanted hardware upon patient request or decide case-by-case, and only 17% refuse. 80% of orthopaedic surgeons support standardized guidelines to allow for safe return of implants to patients. There is strong ethical and medicolegal support for honoring patient requests to return removed implants to the patient/family.

Thanks to Ira Kirschenbaum MD and the Journal of Orthopaedic Experience & Innovation team for featuring our recent The Surgeon's Record presented by Commons Clinic article on evidence-based medicine. Delivering high value treatment by following the evidence is always the goal. But we should always think critically about that evidence and not underestimate the importance of clinical experience. Medicine will always be an art. At the same time, we need outlets for surgeons, startups, and other healthcare innovators to share their results outside of the traditional medical publishing machinery (whose evidence often isn't as solid as we may think). This isn't an anything goes situation. No evidence or weak evidence (spurious white papers "certified" by paid third parties or pay-to-play journals) isn't the goal either. Instead, the article is a call to create a pathway for sharing practical, actionable research. Thanks to Ira and JOEI for leading the way. #medicine #healthcare #ebm #evidencebasedmedicine
