The Little Things That Count A practical and thoughtful gifts -a full-packed first-aid kit for the car that I received at the recent 'Internal Medicine Symposium 2024' organized by Sunway Medical Centre. Unlike souvenirs like pens, mugs, or notebooks, a first-aid kit serves a functional purpose that resonates with a hospital’s core mission—caring for health and safety. It shows that the hospital is not just a place for treatment but also a partner in everyday well-being. Every time the kit is used or even seen in the car, it reinforces the hospital’s brand in the minds of customers, building trust and long-term loyalty. The fight for fair reimbursement isn’t about #independentpharmacy versus #chainpharmacy—America needs both, with each serving its unique purpose. One of my favorite pharmacists, Dr. Mak (aka the “Superman Pharmacist”) so eloquently illustrates this in this video clip. The mistake businesspeople sometimes make is falsely assuming that pharmacy is strictly a transactional business. Pharmacy can be transactional, but often isn’t. While some patients just want to hit the CVS drive-thru, others are more medically complex and need a higher degree of counseling to ensure no contraindications to medications, and/or that they are using their medications appropriately. This becomes increasingly important as we age, where the number of prescriptions we’re taking, potential side effects risk and preexisting health conditions grow. We need independent pharmacy to survive. And we especially need to ensure that we align Medicare reimbursement in such a way as to not disincentivize the type of high-touch, hands-on care that many of these Independent Pharmacies excel at.

One of the most challenging parts of being a physician on call during the holidays is finding the balance between practicing evidence-based medicine and helping patients achieve what matters most to them—especially when those goals involve spending cherished moments with their loved ones. For some patients, this may be their last holiday with family, and we do everything we can to honor their wishes while ensuring they receive the best care possible. Whether it’s attending a special event, participating in a beloved tradition, or simply being home with family, every patient’s goals and priorities are unique. Deciding whether to discharge someone early, delay chemotherapy to minimize side effects during the holidays, or prioritize comfort over the ‘standard of care’ is never straightforward. This is where the art of medicine comes in—collaborating with patients to find a path forward that respects their goals while delivering compassionate, high-quality care. It’s a process that hinges on trust and open, honest communication. As physicians, we work to create a space where patients feel heard, valued, and empowered to take part in the decision-making process. Medicine is about more than just treatments; it’s about people. And during the holidays, that truth resonates even more deeply. Take the time to talk to your doctor, share your goals, and ask questions. Promote independence, through changing the way we assess and provide equipment so that it can be used to enable and prevent deterioration and loss of independence rather than as a reaction or response to deterioration or crisis. Easier to prevent than to regain
