Have you seen our latest episode of "Healthy Days at Home" about the importance of care coordination and the shift to risk-based models? Here's what you're learn by watching the <15 min. video featuring Nick Stupakis, vice president of Helion, and Dr. Vicenta Gaspar-Yoo, the senior vice president of care continuum and transitions at AHN: ⚫ Finding the right post-acute placement for each patient is not a one-size-fits-all approach. The right site can lower cost of care and improve health outcomes. ⚫ Dr. Gaspar-Yoo is focused on improving throughput, care transitions, care management and coordination across Allegheny Health Network. Close collaboration with Helion and its high-performing post-acute care networks is a key component of this strategy. ⚫ Transitioning to fully risk-based arrangements makes operational excellence and patient-centered care more important than ever. ⚫ In 2024, Dr. Gaspar-Yoo is most excited about a new level of blended care coordination for the enterprise across case management, acute, post-acute and ambulatory settings.

Of the many things that made me stop and think at NHS Confederation Expo, by far the most personally impactful was the incredible testimony and appeal from Merope, asking truthfully whether lessons are always learned, and how committed we are as health leaders to de-constructing professional status, giving patients more agency and truly welcoming challenge. From the most painful experience, her simple request to consider 'What if I'm wrong', and to build simple systems to enable people to raise concerns at crucial times is a powerful reminder of the need for change. https://lnkd.in/ebk_bMUz

Any hospital closure is devastating. It’s even worse in rural areas: Patients lose the only nearby option for critical health care needs, and communities lose a major employer. As this Becker's Healthcare piece shows, rural hospitals continue to struggle. But I believe we can change that, if all of us – providers, community leaders, policymakers – work together. We share a common interest – making the places we serve as strong, vibrant and healthy as possible. What’s at stake is nothing less than the future of our communities, and the quality of life for our children and grandchildren. #ruralhealthcare #ruralhospitals

Surrey Memorial Hospital to get new acute-care tower, health officials announce A new hospital tower is always good news, but the good news seems to be lacking some details..... Where is the business plan? What is the building budget? When is the timeline? Where are the engineered drawings? Where are the extra medical professionals to staff? What % of the budget will be covered provincially and municipally? P.S. make sure to get a helipad included. It really helps with closing the time gap on the "golden hour" for air transport emergencies.

It is a sad testament to the state of healthcare in New Yorks State that there are more F rates facilities in New York than anywhere else, and New York does not boast one single A rated facility. I fear the situation will only worsen especially with recent reports that there are more rural hospitals at risk of closure in New York than any other state. NYS leadership needs to step up for the sake and safety of our citizens to provide the healthcare they deserve. https://lnkd.in/er-QZeNP https://lnkd.in/eZhQ7Cp3

With support from CDC’s Division of Healthcare Quality Promotion, NACCHO has supported University of California Irvine’s work to study and promote the use of chlorhexidine (CHG) for routine bathing and showering in nursing homes. CHG, which is a disinfectant and antiseptic, is shown to reduce the extent to which residents in nursing homes or patients in other long-term care facilities (LTCF) are colonized with certain bacteria. In this blog, NACCHO discusses the current role of local health departments in supporting CHG bathing practices, challenges to implementing them, and recommendations for those interested in introducing or supporting implementation of the decolonization strategy in health facilities.
