There is another side of the debate that is rarely discussed but absolutely feasible: "Tell us what you want to pay and we'll tell you what you can have." It's a real economic element that doesn't get enough air time: mismatched expectations. If EMS is willing to do all the things for the dollars offered -- what I've repeatedly at this point called "the niceness problem" -- then there is no sense in the counterparty negotiating because they get it all for less. EMS could choose an alternative path, which is to be clearer around what you can get for your available dollars. Just like you don't get a Mercedes for the price of a Vespa. If you want the Mercedes, then pay for the Mercedes. But if you don't know what the Mercedes costs, then are we expecting payers to simply know, magically, what their number should be? How many agencies have ever EXPLAINED their cost structures to the folks who are supposed to pay for them? #transparency
“Sticker Shock” local governments are increasingly surprised when they see the cost of providing EMS. Most non-government EMS does a poor job of providing information about the financial gaps between insurance receivables and the costs associated with the service. Local governments need to understand that if they aren’t subsidizing their EMS to account for the gap in receivables and expenses, they soon will have to. Ambulance deserts are increasing nationwide, with EMS systems collapsing due to antiquated staffing models, funding gaps, and poor leadership. Unfortunately, for many areas it takes a sentinel event like in this case for action to be taken.

One cannot MEASURE the value of "professional" or "life-saving." Mobile Medicine won't solve its economic problems by begging for cash. It won't solve them by calling things "essential," or raising local taxes or getting a stipend. They cannot be solved by arguing about what the profession should call it itself, or whether degrees should be required. Mobile Medicine's struggles -- including the lack of money, people, even hours to sleep, and poor diet historically (big fan of Jonathan Tate, who is changing the latter!) -- are a DATA PROBLEM. One must be able to MEASURE IMPACT in numbers. One needs EVIDENCE OF EFFICACY, not anecdotes or heartstring-tugs or cries of "no ambulance." The latter won't stick. They might work here or there but it won't overall. NEMSIS Technical Assistance Center is an essential component of the solution, yet NEMSIS gets constantly disrespected. It should NOT. Its datasets are vital and very, VERY misunderstood. Clay Mann , Joshua Legler , Jennifer Correa Every part of healthcare is complaining about cash, hiring, burnout, retention, legal. Mobile Medicine, by and large, still seeks "free" and "pretty" but non-interoperable. There's your breakdown: make your impact measurable, in langauge that payers understand, and money will flow.
