A cautionary tale... I saw a patient with a medical issue. It's a common issue and is important but not urgent. In my usual private practice I would counsel the patient, fire up my referral letter, and in less than 30 seconds, the patient and the non GP specialist would have the referral. I would then have the patient rebook in a month so I could find out what happened/address any problems/continue care. However, I was in my practice where patients use public hospital outpatient more frequently. I had to find a special referral form, append it to my usual comprehensive letter, and then remember how to use email to fax. I also had to task my reception to make sure it landed. I also gave a hard copy to the patient as they were heading in to that hospital on another matter. I rebooked for a month to chase things up should they go awry. The patient arrived a month later. They had dropped off the referral letter, were told they were on the list, and that they would be contacted. They had not been. I called the hospital, three people later, I discovered the patient had an appointment, in 8 days time, with the right people/department, but at another hospital. I asked whether they had told the patient this. ‘we usually send a letter’ I thanked them, informed the patient, and rebooked for a month so I could find out what happened/address any problems/continue care. Why is referring to public hospitals so difficult and opaque? Why are they still using fax machines? Why not healthlink? Why are they not using text messages? Why are they still using snail mail? This is a woeful use of our scant health resources, on both sides of the primary/tertiary divide. I present this not as a whinge, but as a cautionary tale. GPs need to optimise for excellence; we need to follow these things up; this needs to be done in paid time.
