DATA RELEASE: PAINFUL SURGICAL WOUNDS In a prospective Quality Improvement Project conducted by AdventHealth’s general surgery and wound care teams, pain scores and pain medications were reduced by >80% with the implementation of Altrazeal in the management of complex post-operative wounds with severe pain scores (mean = 8/10 VAS). Nursing efficiency was also enhanced as frequency of dressing changes and wound care assessments were reduced by 67%. Average length of stay reduced by 33% from 6 days with SOC to 4 days with Altrazeal leading to a 41% cost savings. All patients healed without any adverse events. 🎉 A Triumph of Compassion and Innovation in Surgery! 🎉 Nice to share a good news story…. When a severely burned heavily pregnant patient crossed the path of Dr Nikki Allorto and her team at Greys Hospital in Pietermaritzburg, every effort was made to achieve what at this stage is a remarkable outcome. This patient, a heavily pregnant woman with more than 60% TBSA burns, required immediate intervention. Despite the severity of her condition, an emergency caesarean section was performed, and a healthy baby boy was delivered. However, the mother's condition remained critical. While standard protocols suggested palliative care, Dr Allorto pushed to find a solution that would give the mother a fighting chance at life – this in the hope that her baby boy would enter life with a mother to care for him. Opting to use MEEK Micrografting Technique for the first time at Greys hospital, Dr Nikki contacted our meek team, who arranged for the equipment and team to be present during the surgery. The MEEK Micrografting Technique, an advanced wound care procedure known for its effectiveness in large TBSA burns. Arriving with the equipment, our Meek team supported Dr Allorto in the application of the Meek technique – and she was able to cover the vast majority of the patients burn wounds in that one surgery. To this point the patient is doing well, and although it is early days, we are hopeful for an excellent outcome. CMS published the final rule announcing the Transforming Episode Accountability Model (TEAM). TEAM aims to incentivize care coordination, improve patient care transitions, and decrease avoidable hospital readmissions for five common surgeries. Participating hospitals will be accountable for the quality and cost of an “episode of care” that includes the surgery and all items and services for 30 days following hospital discharge, such as skilled nursing facility stays and provider follow-up visits. ‼️ This little tool can greatly impact lung function after surgery or illness and prevent lung infections ⬇️ 💡 An incentive spirometer is a hand-held breathing device that a patient uses to take slow, deep breaths through the devices mouthpiece. This exercise for the lungs helps to maintain lung strength and also helps the lungs become stronger after an injury, illness or surgery. ➡️ Patients are instructed to use it at least 10 times every hour while awake. Surgical complications present a significant challenge worldwide. To address this issue, the World Health Organisation (WHO) introduced the Surgical Safety Checklist (SSC) as a means to decrease complications and fatalities during surgery. Nurses play a critical role in ensuring patient safety throughout the operation. A clear understanding of the checklist promotes effective communication within the surgical team, fosters a safety culture, and reduces complications.

WORLD ANAESTHESIA DAY World Anaesthesia Day commemorates the birth of anaesthesia on 16 October 1846. Millions of children are anaesthetised each year. Some are fortunate enough to be looked after in dedicated paediatric centres, undergoing surgery and anaesthesia with fully trained specialists. At the other end of the spectrum there are places where children do not receive surgery because of a lack of trained anaesthetists or suitable equipment. Almost all anaesthetists, particularly those working in developing countries, expect to care for children regularly. This edition of Update in Anaesthesia contains a review of paediatric anaesthesia which we hope will prove practical.
