As an advertiser, you can create Lead Gen Forms if your Ad account is associated with a LinkedIn Page.
You can also create a new Lead Gen Form from the Assets dropdown on the left menu of your account homepage by completing these steps:

Sign in to Campaign Manager.

If you need to switch accounts, click the account name in the upper-left corner of the page and select the correct account name from the dropdown.

Click Assets on the left menu and select Lead Gen Forms.

Click the Create form button at the top.

Complete the sections you’d like to include in your form.

Click Create.
Note: The new Lead Gen Form will be in Draft status if the campaign has already been active. From the Ads tab, click the ... More icon next to the correct ad name and click Edit. Click the dropdown to the right of ad to change the status to Active.
There are five sections of the Create Form page.
Enter the Form name, Language, Headline, and Details.

The Lead Gen Form should match the language you've selected for your campaign.

Form image: Upload or select a custom image for your Lead Gen Form. If you choose not to upload or select a custom image, the image will either default to the ad image (for single image ads only) or the company page image (for all other ad types).

The headline character limit is 60 characters.

The detail character limit is 160 characters with two lines.
In this section, select form fields to identify the information you'd like members to provide.

Click Contact, Work, Company, Education, or Demographic.

Click the checkboxes next to the correct form field names.
- By default, First name, Last name, and Email address are selected on the Lead Gen Form, but can be unselected.
To add custom questions:

Click Add custom question (0/3).

Select the type of question from the dropdown (Single line text input or Multiple choice).
- If you’ve selected Multiple choice, enter the answer options (up to 10).

Type your question in the Question field (maximum 100 characters).

The questions can be arranged by moving your cursor over the Reorder icon clicking and dragging.

Custom question fields are included in the maximum of 12 fields for the entire form.

Members will be required to answer the Custom question(s) to submit the form.
Enter your Privacy policy URL and Privacy policy text.

Privacy policy URL: This web page should be solely dedicated to your privacy policy. A link to the URL will appear on your form.

Privacy policy text: This text should include your primary use description which states what you intend to do with the members information they are choosing to submit on the Lead Gen Form. You can generate multiple hyperlinks from any selected text within your privacy policy text.
Disclosure checkboxes
You have the option of using disclosure checkboxes to get members’ specific consent for any additional ways you intend to use their information.
To add a disclosure checkbox:

Click Add checkbox (0/5).

Add your Checkbox message in the Enter your terms and conditions field. This is the description that you’d like your campaign audience to see next to your checkbox. Within each checkbox, you can add as many hyperlinks as needed, The max character limit is 500 characters.

Click the Required checkbox to make the checkbox a requirement for form submission.

To add another disclosure checkbox to the same form, click Add checkbox again.

Thank you message character limit: 300 characters

Select a URL to connect members to your ebook, website, or other destination of your choice.

Select a call-to-action button to attach to your thank you message from the dropdown. This CTA drives members to your website or content of choice. We offer the following CTAs:
- Visit company website
- Learn more
- View now
- Download now
- Try now
Hidden fields
You can add up to 20 Hidden fields (optional), which allows you to create and track additional fields in your Leads report.

In the Hidden fields section, click
Add hidden field, and complete the fields. - The Field name will be conveyed as a column on your Leads report.
- The Field value will be assigned to every submitted lead in this column on your Leads report.

Add hidden field to add more.

Tips for your destination URL:
- Some special characters (for example: < > # % { } [ ] and others) cause errors when they're included in the query string at the end of the URL. Even if these characters are accepted, they might not be supported in all browsers. If your URL generates an error, copy everything after the question mark into the URL and paste it into a URL encoder such as http://meyerweb.com/eric/tools/dencoder/. Then, copy your results and replace the original query string in the URL. This should resolve the issue.
- If you add URL parameters for third-party tracking, do not use quotation mark characters within the parameter settings to avoid technical errors with reaching your destination URL.
