On Tuesday, our Executive Director, Joseph Conte, joined other Social Care Network (SCN) leaders in the community to discuss the New York State 1115 Health Equity Waiver at the Blooming Health event. The event focused on preparing Social Care Networks for success to serve 7 million Medicaid members across New York.

Honored to present at the National Opioid Congress. The Hotspotting effort utilizes innovative technolgy, contingency management and advanced analytics and the human touch- certified peer recovery advocates to reduce OD in the 650 involved individuals by 80%. This year 10 partners are involved and we expect to reach over 1000 individuals to support their recovery journey. Especially proud that when 1st speaker Samuel A. MacMaster, PhD Chair of the Advisory Board National Institute for Behavioral Health called out SI as the epicenter of opioid epidemic 2015 that the Hotspotting was able to reverse that trend and save lives. #SIPPS #Spectramedix #ReadyComputing #WELLTH

There is no substitute for work happening “on the ground”! The SIPPS, a not-for-profit, has educated and placed over 250 Community Health Workers (CHW) over the past 4 years in all 5 Boros and Nassau and Suffolk County. This was done at no cost to the agencies w funds sourced from grants and philanthropy not venture capital. We are proud of this work and the opportunity to serve those in need. The SCN work before us presents a once in a generation opportunity to demonstrate that social care matters. Innovation, grit and determination will carry the day.
Happy National Community Health Worker (CHW) Awareness Week! This week (August 26th-August 30th) we are celebrating CHWs for their achievements and hard work in the community. CHWs serve as a liaison between health and social services and the community to facilitate access to services. They improve the quality and cultural competence of service delivery.
