Prime Care Coordination began as a means to prepare for the shift to care coordination and managed care for people with intellectual and developmental disabilities. Through the collaboration of providers in local communities. Prime Care Coordination was launched to assist people with intellectual and developmental disabilities with managed care and care coordination. The Southern Tier Prime Care Region held its Annual Holiday Food drive for their location food pantry CHOW (Community Hunger Outreach Warehouse). The Care Management Teams spent four weeks bringing in different nonperishable food items, and overall, the Southern Tier Region collected 670 Pounds of food, which equates to 558 meals for the local community! #FoodDrive #CareCoordination #PersonCentered
