    Everyone has their favorite time of year or special traditions. For me, it has always been Christmas. The season from Thanksgiving to New Year's was filled with celebrations, my birthday, and extra time with my dad, who owned his own business. As a child, I didn’t fully understand the sacrifices he made to provide for our family. What I did know was that the winter holidays were my favorite time with him. Fourteen years ago, my connection to this time of year changed forever. On my birthday, we signed my dad onto hospice care, and he passed away on December 30th. The holidays now bring waves of grief and, at times, anger. I feel mad that my favorite time of year was touched by his death. But I also see the beauty in what we were able to give him: the gift of a comfortable and peaceful final Christmas. His oncologist had told us he would make until spring, but as she walked me out of his hospital room, she quietly suggested celebrating Christmas early. I was furious at the time—she hadn’t been brave enough to tell him the truth about his prognosis. Looking back, I understand her hesitation, but I also know that withholding the full truth robs people of choice. It robs them of the power to make decisions about how they want to spend their remaining days. In my 15 years of working in hospice care, I’ve seen progress in how we talk about death and dying, but we still have a long way to go. Too often, we avoid these conversations during the holidays because they’re “too hard” or “too sad.” But avoiding these discussions doesn’t stop death from coming—it only takes away a person’s chance to plan their final days on their terms. As healthcare providers, we have a responsibility to approach these conversations with honesty, compassion, and courage. It’s not enough to avoid the hard truths. When we are truthful, we empower patients and their families to make meaningful choices—whether that’s celebrating early, saying goodbye, or simply spending intentional time together. This holiday season, I encourage my colleagues to be courageous in these moments. Be honest with your patients. Give them the knowledge and the opportunity to decide what matters most in their final days. It’s a gift only you can give, and it could mean everything to them and their loved ones.
